The Registered Nurse is a member of an interdisciplinary care team. This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation. This position is 1.0 FTE, 40 hours/week. Caregiver Summary: The Caregiver is a member of the home care team who works under the supervision of a Care Manager to provide personal care and support services that meet each client’s individual needs. The Caregiver is assigned to clients in a manner that promotes quality, continuity and safety of care, and delivers services in alignment with HomeWell’s mission and standards of care. The Caregiver does not perform tasks that constitute the practice of nursing, including administering medications or physician-ordered procedures.
