Key Responsibilities Assessment Management: Monitor, guide, and supervise the input and completion of MDS assessments in accordance with state and federal regulations. Care Planning: Schedule and facilitate quarterly and annual care plan conferences involving residents, families, and the interdisciplinary team. Hours would be: 6 : 45pm-10pm or 7:00am-11am RESPONSIBILITIES: Resident Care (PHI) Provide a balance of emotional and physical support that allows an individual to function at their highest level of independence.
