Notifies managing physician about unusual or unexpected events or problems; makes appropriate referrals to other healthcare professionals and support services 3. Dictate and document all patient interactions and services rendered per medical record completion policy 4. Provides education and support to caregivers on dementia, its progression, and coping strategies to include healthy lifestyle behaviors, disease prevention and overall health promotion 5. Support at-home rounding support for home bound patients, nursing or rehab facilities, to provide initial or follow up diagnoses, to include provider referral management for monitoring disease progression and or management.
