Key Responsibilities Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings. Ensure timely assessments, plan of care development, and interdisciplinary team (IDT) meetings for hospice patients. Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards. Oversee staff scheduling, oncall rotation, visit coverage, and staffing models to maintain patient care continuity. Collaborate with the admissions/referral team to ensure smooth clinical onboarding of new patients and transitions of care. Review clinical documentation, patient outcomes, and quality metrics; identify and implement process improvements. Provide leadership in symptom management, endoflife care best practices, patient/family education, and staff education/training.
