This role spans the entire continuum of care, from preadmission to post-discharge, ensuring patients achieve their care goals efficiently and effectively. Key responsibilities include: Coordinating care with the multidisciplinary healthcare team to optimize patient outcomes. Leading case management activities, including staff recommendations, space utilization, and performance improvement initiatives. Overseeing and mentoring case management processes, from assessment and care planning to resource referral, psychosocial counseling, and discharge planning. Ensuring all activities adhere to social work professional standards, hospital policies, and external regulatory requirements, aligning with the hospital’s mission. This leadership position emphasizes efficiency in resource utilization, timely goal achievement, and maintaining the highest quality of care, making it a cornerstone for hospital performance and patient satisfaction. Responsibilities Collaborates with the case management and healthcare team to coordinate processes including oversight of assessment/reassessment, care planning, psychosocial counseling, discharge planning, resource referral, patient education related to social/finance/behavioral issues, LOS management, documentation. Integrates services into the primary function of the department and with services of other departments/functions. Uses collaborative approach in relations with other departments and staff to actualize responsibilities; assures a customer service approach in all activities. Serves as a role model for professional commitment, professional behavior, and effective problem-solving. Identifies staffing needs, orients, assigns and schedules staff to ensure availability of social work services days, weekends, and evenings. Assesses and recommends space and resource needs for case management staff. Provides informal supervision and serves as a resource to all staff on complex and/or long stay cases, entitlement programs, insurance requirements, community resources, guardianship issues, psychiatric commitment, abuse and neglect and hospital policy. Assures development of staff through supervision, staff meetings and educational opportunities; assures completion of annual compliance education and assures skills necessary to age appropriate, culturally sensitive care. Develops/maintains/revises policy and procedure pertinent to social work activities. Participates in organizational committees, task forces and other meetings as assigned, to assist the organization in meeting its goals. Ensures that standards of practice follow corporate compliance guidelines, acts in an ethical and legal manner, and assures that all actions are in the best interest of the hospital. Helps assure satisfactory results of accreditation surveys and licensure reviews by DHSS and TJ. Helps with through put and serves as a resource to other members of the case management teams. Attends rounds on all units to assess outliers to reduce the length of stay while providing patients with the appropriate resources. Qualifications 2-3 years of Case Management experience in acute care setting preferred. Previous leadership experience preferred. Location: Kent Campus Hospital Status: Full Time 80 Hours Shift: Days SALARY RANGE: 151,819.20 - 235,310.40YEARLY General Summary: Works independently and with autonomous collaboration with physician to provide complete physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. The Nurse Practitioner works within a collaborative model to provide quality care and to improve patient outcomes by strengthening continuity of care in all settings for patients, families, and staff in both the hospital and physician practice settings. When department/case applicable, the Nurse Practitioner (NP) remains responsible for the patient throughout the course of the treatment, provides the physical examination, the diagnosis, and the health care plan, determines when the patient has recovered, and releases the patient. The NP may refer the patient to physicians for consultation or to specialized health resources for treatment but retains responsibility for clinical management as well as any subsequent medication. The NP may perform pre, intra, and post-operative care of surgical patients and functions as an essential member of the surgery team. The NP works directly to assist the surgeon in all patient care activities. In addition to job description requirements, incumbents must also meet minimum criteria for medical staff privileges. Responsibilities: Examines patients to determine presence of illness, utilizing physical findings, laboratory results, medical history, ascertains nature and extent of illness and prescribes treatments as indicated. Provide quality care across the continuum in collaboration with physicians, Advanced Practice Providers, nursing staff, and other members of the care team, and be expected to manage patient care autonomously within the scope of their professional practice. Develops plan of care for patients in coordination with multidisciplinary team and completes appropriate reports to physicians and family. May provide inpatient care, rounding on surgery patients and providing pre, peri and post-op services including (A) Caring for complex surgical population (B) Assisting responsibilities in the operating room (C) Ordering and collecting test samples and assessing to update patient's plan of care as necessary (D) Educating patients and families during patient's hospital stay (E) Taking appropriate action when medical complications arise (F) Within the scope of their education, training, and credentialing, provide full-scope care for patients in the inpatient setting Orders and reviews appropriate laboratory and diagnostic procedures. Reviews all lab data and documents. Issues prescriptions to patients based on Federal & State Regulations. Documents and maintains patient records of services provided according to facility and nursing standards. Provides direct clinical care for the patient population according to established protocols and utilizes evidence-based practice standards in collaboration with the team. The NP provider will participate in educational activities, and research as applicable, ensure regulatory compliance, document care according to standards, and prescribe by their collaborative agreement and State regulations Provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care. Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR). Participates in the performance improvement processes to meet the quality of care and service standards. Participate in and support accreditation, compliance, and regulatory activities of the organization. Provide agreed upon coverage to care for patients in the appropriate settings (ex. ambulatory office and/or hospital) during business hours and evening/weekends, as mutually agreed upon and per medical staff bylaws.
