Key Responsibilities Maintain and/or create patient accounts and records within the medical software program. Assess the accuracy of demographic and policy information within patient accounts. Photocopy patients' identification and/or insurance cards as appropriate. Review provider schedules, screen patient financial/demographic information for accuracy, and update patient records as needed. Review patient insurance policy information. Ensure that clients do not have another payer source such as Medicaid, Medicare, ADAP, and any other third-party insurance. Refer patients to eligibility coordinators in Legacy Clinics within close proximity to the school-based clinic location. Complete all necessary paperwork. Answer incoming calls and route them appropriately. Perform client intake and notify appropriate staff of the client’s arrival. Follow appointment procedures as set by management. Complete opening and closing procedures. Keep the clinic lobby straight and orderly. Directly assist the school-based clinic nurse practitioner with clinic operational tasks, including registration of patients, triage and vital signs, telephone management, patient education and assistance, and other clinic tasks according to procedures, policy, and protocols. Work with the team in ongoing clinic development and implementation as needed. Document all client care and contact appropriately and in a timely manner. Assess the needs of clinic clients and parents and assist as directed by the nurse practitioner and per protocols and procedures. Consult with clients by telephone and provide follow-up, feedback, and assistance as needed. Participate in clinic public health education and registration activities. Assist with follow-up of lab results and other testing results, including follow-up of abnormal values as appropriate and directed by the nurse practitioner. Process lab specimens as assigned by the Medical Director of Pediatrics. Perform phlebotomy duties as needed. Assist other providers in the future as needed due to clinic expansion. Assist with administrative tasks necessary for the successful operation of the clinics. Participate in performance improvement activities and performance reviews by the Director of School-Based Operations and Medical Director of Pediatrics. Perform other duties as assigned. Minimum Qualifications High School Diploma or GED, some medical knowledge. Must maintain BLS/CPR Desirable Education and/or Experience Completion of Medical Assisting program at an accredited school. Certification preferred. Required Knowledge, Skills, and/or Abilities Ability to work independently. Ability to multitask. Ability to function and communicate with parents, students, teachers and team members who have a wide range of educational and social backgrounds. Implement preventative actions to reduce incidences of pressure ulcers and vascular ulcers. Provide instructions on proper wound dressing and care. Educate the patient, patient’s family and other support persons on ulcer prevention and treatment. Works collaboratively with the multidisciplinary team to provide a comprehensive plan of care. Design pouch systems for draining wounds and fistulae. Review and modify pouch system problems. Provide comprehensive wound management, including reducing causative factors, controlling odor, assessing nutritional needs, and providing and applying topical therapies. Provide education and support for individuals who undergo surgery for colostomies, ileostomies, or urostomies. Take actions to prevent and address peristomal skin complications. Select and recommend collection devices/undergarments. Maintain data and reports and requested. Participates in quality improvement. Demonstrates the knowledge and skill necessary to provide car appropriate to the age of the patients served. Lead prevalence study. Develop practice standards for managing patients with wound ostomy and incontinence care needs. Monitor patient outcomes on an ongoing basis. Create education programs specific to wound ostomy continence patient care in collaboration with the clinical education team. Provides education programs to staff. Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
