Job Summary: Collaborates with Education Consultants, Managers, CNS staff and Physicians to develop, design, deliver and evaluate/improve education strategies and special projects/programs based on clients departmental goals and needs, as well as organizational goals/objectives. Implements education programs and strategies and coordinates to meet regulatory program requirements and organizational needs. Participates in competency assessments and evaluations of new and existing staff. Contributes to ongoing employee performance appraisals. Evaluates performance issues and develops strategies for remediation. Responds to corporate and industry changes in clinical practice and professional education and provides necessary training, education and development which improve organizational performance. Participates in leadership projects, meetings, committees and improvement activities. Ensures compliance and documentation of organizational records based on regulatory agency standards. Overview The RN is a professional caregiver who assumes responsibility and accountability for patients having a procedure performed in the Operating Room. The RN has advanced training in this area and provides care utilizing the nursing process, therapeutic environment, treatment and diagnostic processes, and interdisciplinary collaboration. Qualifications Current licensure as a Registered Nurse in the State of California. Current Basic Life Support Level C (Healthcare Provider) certification required. The Licensed Vocational Nurse (LVN), under the direct supervision of the Registered Nurse is responsible for identifying the needs of each patient and assisting in the planning and the delivery of patient care to meet those needs. Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 7am to 3:30pm, 5 days on, 2 days off. Sunday - Thursday. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Job Description The Medical Coordinator is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $30.00 - $35.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3 graveyard shift differential. Schedule is 3pm-11:30pm, Tues-Sat. Job Requirement Minimum Experience Requirement: All (1) newly graduated nurses, (2) re-entry nurses, and (3) nurses new to the U.S. healthcare system must satisfactorily complete the Emanate Health R.N. New Grad Program. One year critical care experience is preferred. Excellent customer service skills required. Minimum License Requirement: Current RN license in the State of California. BLS certification required. ACLS required within three (3) months of hire/transfer. National Institutes of Health (NIH) Stroke Scale Certification "A" required within three (3) months of hire/transfer for those working at ICH and FPH. Annual one (1) hour of Stroke Education required for those working at QVH. Must be available to work every other weekend Delivering world-class health care one patient at a time. Overview The staff RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the therapeutic environments, treatment and modalities and interdisciplinary collaboration. Responsibilities The RN shall, for their patient, collaborate in the formulation of the overall plan of care for the patient, as evidenced by written documentation. Identifies needs and problems specific to biophysical, cognitive, developmental, emotional, socio-cultural, and spiritual. Demonstrates accountability for on-going awareness of the status of assigned patients. Collects and documents data appropriate to the nature and severity of the condition. Qualifications Current CA Registered Nurse license. Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Basic Dysrhythmia certifications are required. NIHSS certification, and management of assaultive behavior is required within 90 days of employment.
