JOB SUMMARY Responsible for performing the daily activity calendar requirements set by the Activity Director. May prefer bi-lingual abilities based off of individual facilities. Must have a valid, IL driver’s license. Must be punctual and time efficient, especially as it relates to following the activity calendar. Possess the qualities of empathy, compassion, understanding, enthusiasm, personable, energetic, positive, and humorous in order to effectively interact with residents, families, and staff. Must be able to relate and work with ill, disabled, elderly, emotionally upset, and at times hostile people. Must be able to communicate effectively with to the community, other health professionals, outside professional organizations, senior services, government agencies, and any other provider or professional in the community. Understand and be able to manage emotional needs of yourself and the staff. PHYSICAL REQUIREMENTS Must be able to push, pull or carry 50 pounds. May be required to lift equipment, supplies, and occasionally move furniture. Must be able to move intermittently throughout the workday. Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents, family members, personnel, and support. ESSENTIAL FUNCTIONS Provide feedback to Activity Director for monitoring achievement of facility and program goals through a team approach. Lead a therapeutic, person-centered activity program, which constantly strives to enhance the quality of life of each resident and meets the Activity Mission and Philosophy. Follow all Activity Policies and Procedures and Federal and State regulations. Become knowledgeable of each individual resident’s background, cultural, life history, disease, and medical care needs in detail to ensure an appropriate person-centered activity plan. Activities shall be daily and shall reflect the schedules, choices, and rights of the residents. Be able to understand communication needs of residents, such as language services, communication boards, etc. Per direction of the Activity Director, conduct, oversee, and complete initial and all on-going assessments, including but not limited to social, medical, cognitive, communication abilities, spiritual needs, physical, neuro-psychological, behavioral, amongst others as it relates to individual activity/leisure. Document and complete all required documentation per Activity Director. Maintain the following records: Resident Daily Participation, Resident Progress notes, Leisure Interest Survey, and Activity Planning sheets to assist in scheduling and meeting resident needs. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Must maintain equipment and supplies in good clean order. Must notify Activity Director within 48 hours if any new materials are needed for scheduled activities and when repairs are needed. Must update calendars and boards daily. Perform other duties as assigned. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, and provides a consultation to meet the patient's goals and improve quality of life. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Provides palliative care consultations in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages advanced illnesses by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care, when appropriate. 4. Educates patient/family on disease progression, trajectory, and options for care. 5. Reviews clinical information, coordinates and oversees patient care in consultation with the palliative care interdisciplinary team and other healthcare practitioners. 6. Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members and other healthcare providers. 7. Reviews and develops advanced directives with patient/family. 8. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. Communication 1. Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care. 2. Consults with the patient's primary care physician, specialists, home health care team, caregivers, and other healthcare providers within the patient's care team. 3. Communicates with other community health practitioners to coordinate the plan of care. 4. Attends and participates in palliative care interdisciplinary group meetings. 5. Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. 6. Educate/counsel patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and palliative care interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 10. Serves as a nursing resource for consultation and educations to members of the interdisciplinary team and other healthcare practitioners in the community. 11. Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Additional Duties 1. Collaborates with other members of the health care team to resolve patient care problems and enhance care delivery. 2. Assists in the preparation for GI cases assuring that supplies, equipment, and instruments are available. 3. Assists the team in the transport, transfer, and positioning of patients in the procedure rooms. 4. Assists with the maintenance, care, and cleaning of equipment and work environment. 5. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
