Responsibilities: Provide direct patient care under the supervision of registered nurses and physicians Assist with daily living activities such as bathing, dressing, and grooming Administer medications and treatments as prescribed by physicians Develop and implement individualized care plans in collaboration with interdisciplinary team members Monitor and report changes in patient condition to healthcare team members Requirements: Current LPN licensure in the state of Pennsylvania Minimum 1 year of experience in long-term care or rehabilitation setting Ability to work effectively in a fast-paced environment Strong communication and interpersonal skills We consider all applicants for positions without regard to race, color, religion, sex, national origin, citizenship, age, mental or physical, disabilities, veterans, or any other protected status.
