Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator or Patient Relations as needed. 6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 7. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. 8. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Professional Development 1. Contributes to the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. 2. Advances clinical competence in nursing practice to progress from novice to expert. 3. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Assesses, plans, oversees and evaluates the appropriateness of care throughout admission and hospitalization of the patient. Job Responsibility Facilitates patient management throughout hospitalization. Performs concurrent utilization management using evidence based medical necessity criteria. Submits data to management regarding case management and/or quality initiatives. Serves as an in-patient liaison - planning, assessing, implementing and evaluating patient in collaboration with the health care team. Conducts chart review for appropriateness of admission and continued length of stay; Facilitates throughput. Conducts clinical reviews and interacts with third party payers to obtain approval of hospital days, pre-certification and post-discharge eligibility in relation to clinical course. Documents the case management process in the medical record; completes and documents a care coordination assessment on the patient. Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Baccalaureate Degree in Nursing is required. Masters in Nursing or health related field is preferred. Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Certification in specialty area preferred. We are seeking to hire Registered Nurses whose direct support, knowledge, skills, and expertise will enable our company to grow, adapt, and expand into new areas of home-based nursing services. This infusion field nurse would be responsible for conducting patient visits to initiate patient care, performing follow-up visits as needed, and precepting and evaluating the skills of newly hired nurses as requested. The Infusion Homecare Nurse will partner with our Infusion Intake Team to ensure seamless initiation of infusion care in patients' homes. Qualifications: Applicants must have at least 3 years of full-time, or equivalent part-time, experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility Supervisory or managerial experience PREFERRED. Infusion experience is REQUIRED. MUST have experience infusing IV/SQIG and biologic medications! Knowledge of principles & practices of supervision, performance evaluation, and education. Knowledge of Federal HIPAA Regulations Knowledge of standard nursing policies, procedures, methods, and techniques. Ability to read and interpret clinical documents, including physicians' orders and treatment plans. Ability to participate in a multi-disciplinary health care team.
