Under the supervision of the Registered Nurse and Program Director, and as a member of an inter-disciplinary treatment team, assumes case management responsibilities for assigned caseload of individuals who have mental illness and substance abuse disorder. Renders nursing care as assigned by registered nurse in accordance with recognized nursing techniques, procedures and established standards based on the scope of practical nursing. GENERAL JOB SUMMARY An exempt clinical position where the nurse practitioner (NP) is responsible for providing direct patient care. The NP participates as a leader of the skilled nursing facility (SNF) care team. Visits managed care and fee-for-service patients at skilled and long-term levels of care in designated SNFs facilities. Provides appropriate evidence-based geriatric medicine. Coordinates care with hospitalists, primary care physicians and care managers. Makes home visits as directed by the medical staff to meet patient needs and provide continuity of care. ESSENTIAL JOB FUNCTIONS Maintains privileges in multiple Nursing Homes as directed by ACA Maintains license and malpractice insurance Consults supervising attending as needed Documents patient visits electronically at least 90% of the time Participates in documentation and other quality improvement programs Available via phone weekdays 8am- 7pm and when on call. Will reviews, approves, and modifies admission orders Creates a detailed admit note for each admission within 24 hours of patient admission to SNF, including medication reconciliation Initiates/documents Advanced Directives Determines if Health Care Proxy status is correct and invoke if appropriate On weekends, takes call for admissions and see new patients within 24 hours of admission on a rotating basis with other practitioners in the program. Daily Visits Initiates and review orders, including medications, on a daily basis Reviews labs, radiology reports, and consults on all patients Talks to and examines each assigned skilled-level patient on daily rounds Monday through Friday Writes at least one daily progress note for each skilled patient Assess patient’s medical stability daily. Description The Medical Assistant II provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Mentors and assists less experienced staff members. Responsibilities Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. Maintains a safe, secure, and healthy work environment by following standards and procedures and in compliance with legal regulations. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms as necessary. May be required to perform proficient phlebotomy duties. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. May maintain, adjust and confirm patient office schedule. May check and verify & update insurance information utilizing internal or web sources. May perform and verify pre-certification for procedures as necessary Provides training and mentoring to Level I MA staff. Directs and assigns MA schedules and work flow to meet the needs of the practice and applicable to patient volumes. Oversee the successful delivery of clinical office support to meet the needs of each physician. Performs other duties and tasks as required. Key Responsibilities: - Assist clients with daily living activities, including bathing, dressing, grooming, and meal preparation. - Provide companionship and engage clients in meaningful activities. - Perform light housekeeping tasks to maintain a clean and safe environment. - Monitor and report any changes in clients' health status to the care coordinator. - Follow individualized care plans and provide care with compassion and respect. - Ensure clients' comfort and safety at all times. - Maintain accurate and timely documentation of care provided. Qualifications: - HHA certification in the state of New Jersey. - Previous experience in home care or a related field is preferred. - Strong interpersonal and communication skills. - Compassionate, reliable, and dedicated to providing excellent care. - Ability to work independently and as part of a team. - Valid driver's license and reliable transportation are preferred. The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities: Following the care plan, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes Assumes responsibility for assignments given, seeks supervision appropriately, and is accountable for his/her actions by performing within the limits of his/her education and experience Performs basic nursing procedures such as taking vital signs, collecting specimens, and monitoring patient conditions. Administers medications and treatments as prescribed by the physician and performs nursing procedures and techniques within scope of practice. Use equipment and supplies effectively and efficiently Utilizes medical terminology to accurately document patient information Lift and transfer patients safely using proper body mechanics Provide acute care for patients with various medical conditions Administer tube feedings and monitor nutritional status Assist with cardiac catheterization procedures Skills: - Valid LPN license in the state of New York - Proficient in suture removal techniques - Knowledge of dialysis procedures and equipment - Experience in infant care and knowledge of developmental milestones - Competent in airway management techniques and CPR certification - Familiarity with medical terminology and documentation practices - Understanding of ophthalmology procedures and post-operative care requirements - Ability to safely lift and transfer patients using proper body mechanics - Experience in acute care settings, including monitoring patients' conditions and responding to emergencies - Proficiency in administering tube feedings and monitoring nutritional status - Knowledge of cardiac catheterization procedures and post-procedure care Note: This job description is intended to provide a general overview of the position.
