This individual will be responsible for care & services to patients as needed. Candidates must hold a valid LPN License in the state of New Jersey in order to be considered. This position will be 7PM to 8AM and will be 1:1 for patients. Responsibilities: Care for patients in the location as needed. Assist with activities of daily living. Follow care plans and deliver the highest level of care. Document medical information as required. Collaborate with interdisciplinary team. Communicate with families and patients.
