Triages phone calls to determine acuity to assist in scheduling appointments. Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality. Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues. Utilizes standard precautions including, appropriate isolation techniques. Consistently follows the Ambulatory Health Care National Patient Safety Goals. Through the model of Shared Governance, collaborates with all members of the health care team with a focus on promoting evidence-based care within a healing culture for the patient population served and their families. Demonstrates knowledge of documentation guidelines. Documents patient’s response to the ordered treatment protocol. Reports changes in a patient condition to the clinician immediately. Position Summary: We have an exciting opportunity to join our team as a Family Nurse Practitioner in Clinical Research in Manhattan. In this role, the successful candidate will be responsible for the provision, coordination, and assurance of comprehensive nursing care using nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of therapeutic, corrective, and prescriptive measures of the designated inpatient or outpatient patient population within a collaborative practice agreement established with a specific physician on the staff of NYU Hospitals Center Job Responsibilities: Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for the service. Assists in the preparation, implementation, and evaluation of research protocols when applicable. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Serves as a clinical expert and resource for the education of peers and other health professionals. Maintains current expertise in area of practice. Promotes own professional growth and development in clinical and managerial role. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. Functions as a respectful member of the health care team. Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities. Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MDs etc. Provides consultation about the therapeutic regimen to the nursing staff responsible for the patients care in the hospital setting. Works collaboratively with nursing leadership to ensure timely communications with the nursing staff. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, housestaff, other disciplines and ancillary services. Participates in the development and revision of interdisciplinary and nursing standards, patient and family education materials as relevant to area of practice and expertise. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members. Minimum Qualifications: To qualify you must have: Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York. Education: Master's Degree in Nursing. Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice. Basic Life Support Certification through the American Heart Association.. Required Licenses: Registered Nurse License-NYS, Nurse Practitioner-NYS, Basic Life Support Cert Preferred Qualifications: Advanced Life Support certification (ACLS, PALS) through the American Heart Association or Neonatal Resuscitation certification (NRP) through the American Academy of Pediatrics may be required based on your selected privileges. At least five years of clinical nursing practice in the care of the patient population served; ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and satisfaction. Previous experience as a nurse practitioner is preferred. Licensed Practical Nurse - Ophthalmology - Nursing - Mount Sinai Hospital - Full-Time

Description The practice of nursing as a Licensed Practical Nurse is defined as performing tasks & responsibilities within the scope of supportive & restorative care; medication administration and specialized tasks under the direction of a Registered Professional Nurse of licensed or otherwise legally authorized physician or dentist. Responsibilities A. Clinical/Technical/Service 1. Participates in the application of the nursing process to an assigned group of patients. 2. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse 3. Performs job duties under supervision of a Registered Professional Nurse. 4. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. 5. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. 18. Participates in health teaching to patients and families following the RNs development of the teaching plan. 19. Establishes an environment that promotes optimum physical and emotional comfort. 20. Performs purposeful hourly care checks and supports the patient experience in accordance with Relationship Centered Care Professional Practice Model 21. Organizes supplies and equipment necessary for carrying out the plan of care is identified and communicated by the charge nurse/designee. 22. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Qualifications A. Completion of educational program for licensure as a LPN. B. Successful completion of required New York State Department of Health LPN IV Therapy Initial Program for the demonstration of competence in the performance of intravenous therapy/blood administration procedures as part of Centralized Orientation. C. Successful completion of annual unit-based review & update program in procedures associated with intravenous therapy.
