JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8. Basic computer skills to document in electronic medical records (EMR) 9. At least 18 years of age. 10. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Job Details The Department of Hospital Medicine is looking to add a qualified Family/Adult-Geriatric/Acute Care Nurse Practitioner or Physician Assistant to join the Hospital Medicine team. In this role, the APRN/PA will work at the top of their license caring for patients at Cleveland Clinic Main Campus. This new role provides night coverage to admitted patients at Main Campus under the Hospital Medicine Service. This night shift position is 15 hours (4:30pm to 7:30am), and APRN/PA will work approximately 11 shifts per month. Responsibilities in this roles include performing admissions, in-person evaluation, and nursing cross-coverage support (Rapid Response and Changes in Clinical Condition). Ideal candidates would have at least 1 year of inpatient experience, but new graduates are encouraged to apply. Responsibilities: Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Must be able to work with collaborating physician. Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board. Performs routine procedures such as injections, immunotherapy, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury which may include assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life-threatening situations, as privileged by CCF Credentialing Board. May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing/Medicine and CCF policy. Performs specialized procedures, including bone marrow aspirations, biopsies and harvest, blood draws, designated IV starts, spinal taps, thoracentesis and paracentesis as well as other specific procedures as directed by the physician, following approval by CCF Credentialing Board. Provides information and answers routine questions of patients regarding their disease, including preventive health maintenance topics such as diet, weight, smoking and topics related to medical specialty, with basic recommendations. Educates patients regarding their disease, treatments, related drug and treatment side effects and hazards. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment. Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary. Performs research follow-up studies. Assists in data analysis. Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care. Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management. May serve as clinical preceptor for PA/APRN students and other health professionals. May serve as Faculty for onboarding new employees. Demonstrates a high standard of moral and ethical behavior. Education: APRN Effective 05/01/2014: MSN required upon hire or within 3 years of hire for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
