Job Description The Ambulatory RN Care Coordinator is a vital member of the healthcare team responsible for ensuring seamless and high-quality care for patients in an ambulatory setting. This role involves both direct patient care and administrative responsibilities, with approximately 60% of time dedicated to patient-facing duties. The RN will support ambulatory physician clinics by seeing patients with complex and chronic disease that require additional support during outpatient visits. They will be involved in working with patients to develop and execute care plans, provide patient education, and work collaboratively with interdisciplinary healthcare team members to optimize patient care and experience. Job Description The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. Responsibilities The Nursing Process Assessment - Demonstrates the nursing skills to complete a holistic assessment of the patient’s physical, psychosocial, spiritual, and education needs utilizing The Christ Hospital Professional Practice Model. Understands signs and symptoms and data related to the patient’s presenting needs and actual or potential nursing diagnosis. Recognizes and appropriately reports to the physician and charge RN pertinent abnormal data and changes in the patient’s condition. A MSN or DNP are not required but supersede the requirement for the BSN. The major specialties treated in this ICU are Medicine, Surgical, Neurological and Cardiac and Donor Care patients. The patient population specializes in critical condition, with most patients generally stabilizing within 24-48 hours following admission. Patient care focuses on stabilizing cardio-pulmonary functioning, maintaining adequate circulatory volume and perfusion, preserving the patient’s protective mechanisms and controlling pain. This short stay ICU will receive patients from ER, outside hospitals and other ICUs as needed to support hospital capacity.
