Develops, maintains, or modifies the individualized treatment plan of care. Executes the treatment plan as developed by the provider and interdisciplinary team members. Summary GENERAL SUMMARY The Nursing Care Partner works under the direction of the Registered Nurse (RN) and provides direct patient care for assigned patient. As a member of the direct care team, the Nursing Care Partner is responsible and accountable for complete and accurate collection and recording of data, completion of routine and delegated direct patient care activities and communicating and collaborating with the RN to prioritize workload. Reports to department leadership. ESSENTIAL DUTIES AND RESPONSIBILITIES Provides developmentally appropriate care to assigned patients. Supports with the patient admission and discharge process. Assists patients with activities of daily living to include but not limited to bathing, foley catheter care, changing, feeding, ambulating patients. Functions as a One-to-One (1:1) Observer (if applicable training will be provided). Collects and monitors data to include vital signs, intake/output, and growth measurements. Maintains accurate records of patient care activities. Reports patient's status and data to the RN. Assists with and/or performs venipuncture for labs as necessary (If applicable, training provided). Completes non-clinical duties to include but not limited to patient transport, stocking of supplies, and cleaning of equipment. Performs all other duties as assigned. LICENSES AND/OR CERTIFICATIONS Required Licenses and/or Certifications Cardiopulmonary resuscitation (CPR) training designation is required at time of hire or must be obtained within 45 days of hire. Preferred Licenses and/or Certifications None preferred. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS Required Education and Experience At time of hire, must meet one of the following requirements: Successful completion of one semester/quarter of clinical rotation in an accredited nursing program. Recent (within 2 years) and previous CHKD Nursing Care Partner experience. Successful completion of CHKD’s nursing student extern program. Graduate of an accredited nursing program (without successful completion of the NCLEX board exam). Active Nurse Assistant Certification, Medical Assistant Certification, Emergency Medical Technician Certification, and/or Patient Care Technician Certification. At time of hire, a record of completion for all education, licenses, training and/or certifications is required. Continuous certification and/or maintenance of state licensures, certifications, training, educational and application requirements is required during employment. Preferred Education and Experience None preferred. Required Knowledge, Skills, and Abilities Exceptional patient care skills with the ability to understand and follow instructions relating to basic patient care.
