Develops and maintains relationships with community leaders, community-based organizations, faith-based organizations, health institutions, schools, daycares, etc. Attends community-based events, including health fairs, support group meetings etc. Assists with the planning and execution of forums, special events and presentations to reach and educate the community. Supports reporting and paperwork to support funding and grant requirements, including tracking patient metrics such as appointment no-shows and lost to follow up ratios. Tracks all outreach and navigation activities and provides monthly reports. Demonstrates an awareness of self-responsibility and accountability for own professional practice and development. Direct charting in his/her shift and make monthly detailed evaluation of all resident charting so that charts reflect progress and condition of residents in the EMR system. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system).
