Responsible for overseeing the implementation and maintenance of targeted treatment teams (or “panels”) at each program, ensuring that clients are cared for by a cohesive multidisciplinary treatment team (psychiatry, counselors, RSS, licensed clinicians, nursing, peers, etc.). Responsible for overseeing the development of the clinical schedules for group programming and ensuring that all programs are adhering to the set schedule and standard evidence-based curriculum for clients based on their phase of care. Provide onboarding training to new clinical staff as well as provide ongoing refresher trainings to clinical team in evidence-based practices including but not limited to motivational interviewing, CBT, trauma informed care, etc. Participates in compliance adherence, audit preparation, and assists the CMO and compliance teams with reviewing data within the program and evaluating program outcomes. Work in conjunction with the compliance lead at each location to ensure we are meeting internal and external compliance requirements. The APP will primarily focus on vascular surgery admissions, inpatient consults, daily floor management, pre-operative and post-operative care, clinical assessment, discharge planning, and coordination of care in collaboration with the vascular surgery team. Responsibilities 1. Inpatient Patient Evaluation & Management Perform comprehensive histories and physical examinations on vascular surgery patients. Evaluate new inpatient vascular surgery consultations and assist with developing appropriate treatment plans. Perform daily patient assessments and manage the ongoing care of vascular surgery patients on the floor. Monitor patients for changes in clinical status and promptly identify potential complications.
