Description The Per Diem Clinical Oncology Nurse Navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge, who provides individualized assistance to the patients, families, and caregivers through informed decision making, timely access to quality health and psychosocial care, and coordination of appropriate follow-up visits throughout the cancer continuum. This individual helps overcome healthcare system barriers, influence the systems and behaviors, and address the gaps of care through research and evidence-based practices guided by regulatory committees and professional organizations. The Per Diem Clinical Oncology Nurse Navigator supports the organizational strategies to deliver high-quality care to the population served through community needs assessment, navigation program development, marketing and outreach, and ONN specific-outcome metrics (i.e., care coordination, guidance, education, and advocacy) established by the organization. The Per Diem Clinical Oncology Nurse Navigator initiates, develops, and implements mechanisms for effective collaboration with the interdisciplinary team, that includes (but not limited to), new patient coordinators (NPC), clinical oncology associates, clinical coordinators (RNs), clinical infusion nurses, advanced practice providers (NPs & PAs), physicians, social workers, and nursing/administrative leadership for the attainment of the highest standards of patient care. Responsibilities PATIENT CARE Delivers patient care by incorporating the tenets of the professional practice model of Relationship-Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Organizes and leads in the execution of day to day patient care coordination, as appropriate to program objectives and areas of clinical expertise. Assists patients in accessing cancer care and navigating health care systems using a thorough chart review in EMR systems. Assesses barriers to care and engage patients and families in creating potential solutions to financial, practical, and social challenges. Identifies appropriate and credible resources responsive to patient needs (practical, social, physical, emotional, spiritual), taking into consideration reading level, health literacy, culture, language, and the amount of information desired. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of a robust plan of care throughout the cancer care continuum. Establishes and implements short and long term goals, objectives, policies and operating procedures to ensure a high level of performance in nursing care delivery to the specialty patient population. Educates patients and caregivers on the multidisciplinary nature of cancer treatment, the roles of team members, the promotion of health, and what to expect from the health system, while anticipating needs and readiness to learn. Follows up with patients to support adherence to the agreed-upon treatment plan through continued non-clinical barrier assessment and referrals to supportive resources in collaboration with the interdisciplinary team. Qualifications Bachelor of Science with a major in nursing required. Master’s degree preferred 3-5 years minimum demonstrated clinical and administrative expertise in the area of nursing practice assigned. 2-3 years’ experience in Oncology Licensed as a registered nurse with current registration in New York State. Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice. Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care. Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic. Utilize standardized protocols for medication management, prescription renewals and prior authorizations. Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner. Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency. Performs other related duties as assigned.
