The Home Care Services Program (HCSP) is a Medicaid funded program that provides long term care to medically fragile and vulnerable individuals who require assistance with activities of daily living through the Community Alternative Systems Agency (CASA), Managed Long Term Care (MLTC) Program, the Assisted Living Program, Care at Home Program and Homebound Medicaid. The Medicaid Eligibility Unit is responsible for the Medicaid eligibility review and processing of initial applications, change actions, and renewals for all Medicaid funded long term care programs. This unit is responsible for the processing of Medicaid eligibility for consumers who receive services through one of the HCSP programs. Home Care Services Program (HCSP) is recruiting two (2) Staff Nurses, to function as Staff Nurses in HCSP/ CASA 11 – Care at Home/ Home Bound Unit, who will: - Conduct in-home or virtual assessments at the request of HRA using a State approved assessment instrument to evaluate the client’s ability to safely perform activities of daily living and the degree to which the client requires assistance for each function and task. The completed assessment would be entered into the New York State Department of Health (NYSDOH) and/or HRA computer system. - Assist in determining the level of home care services or other services of care that the client needs based on the medical diagnosis and degree of functional impairment. The development of a plan of care that takes into consideration client’s home environment and social support system. - Complete in-home and virtual visits and submit the completed HRA/NYSDOH approved assessments and recommendations forms no later than five (5) workdays after the date of assignment. - Conduct case reviews of new applicants, service change and re-authorization cases, and participate in special projects. RESPONSIBILITIES : The Case Manager is responsible for case management services for clients in transitional housing. The Case Manager will have an assigned caseload and will be responsible for the successful integration of both the housing and employment goals of each client. Commit to using evidence-based practices (EBP); Motivational Interviewing (MI), Trauma-Informed Care (TIC), critical time intervention, and boundary setting, to help clients achieve their goals. Conduct orientation with clients and review the facility rules, clients’ rights and responsibilities, grievance/complaint process, emergency disaster procedures, etc. Develop an Independent Living Plan (ILP) for each client, which will define the client’s goals (in conjunction with the transitional housing income building specialists, housing specialists, child care staff and recreational staff). Assist families in obtaining income and public benefits (i.e., Public Assistance; SSI, etc.). QUALIFICATIONS : Bachelor’s degree in social work or a related field required. One (1) year social service experience or approved equivalent. Ability to work effectively in a team environment. Description A nursing student, who has completed the first clinical year, inclusive of medical – surgical nursing, within a baccalaureate nursing program and has a fundamental knowledge of the art and science of nursing, can function in this role. This experience assists the student nurse in expanding previously learned theory and skills through supervised patient care within a work setting. Responsibilities 1. Performs and records patient’s vital signs, such as blood pressure, pulse, temperature, respirations, pulse oximetry and pain rating 2. Description A Senior Nurse Practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner/specialty coordinator provides clinical expertise and clinical program management for the provision of acute/primary health care to patients in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Qualifications Education Requirements Master's degree program designed to prepare a registered nurse to provide acute/primary health care services or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate.
