CPR certification is recommended (required for direct patient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation, valid driver’s license, and state-required liability auto insurance. Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Under direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care and perform bladder scans. Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation. Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Processes daily patient charges and/or related procedures through the system. JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
