They will coordinate the care of patients under the supervision of a Board-Certified Physician. In addition, the Family Nurse Practitioner acts as a member of the clinical team and works collaboratively with other departments in providing patient care. Essential Functions: Delivers direct primary care and preventative services to assigned children, adolescent, and adult patients. Develops therapeutic plans of care for prevention, management, and maintenance of good health for patients. Provides health education and counseling patients. Provides care for Acute and Chronic Illness and Urgent Care services, which includes assessment, diagnosis, treatment, and monitoring of illness. Works closely with multidisciplinary teams and instructs and assists other team members in management of patient care. Follows established practice standards and current evidence-based clinical practices and guidelines. Provides care related to prenatal/postnatal services and care for uncomplicated pregnancies, in conjunction with physician partners, as delineated and approved for in request for clinical privileges. Provides family planning counseling and services. Consults with physician partner and refers patients with more complex/dynamic problems to subspecialty care, as appropriate. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within the specified amount of time expected by the Clinical Director. Ensures that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment. Attends trainings, weekly Provider and Team meetings, and collaborates on practice, administrative, and quality improvement projects, as assigned by Clinical Director Provides clinical supervision and mentoring to Nurse Practitioner, Medical, and Physician Assistant students by mutual agreement with the Clinical Director. May work a fluctuating schedule, including days, evenings, weekend shifts, and holidays according to schedule approved by Clinical Director. Worcester Rehab is a 146-bed skilled nursing facility with a large SUDS population. $45 to $50 an hour, depending on experience. You plan, organize, and direct the MDS process involving overseeing resident care plans through clinical assessment, review of resident's medical history, personal interviews, and completion of MDS reports. Experience & Education: Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university. Must possess a current, unencumbered license to practice as an RN in this state. Prior experience in MDS required. Duties & Responsibilities: Coordination of MDS process Oversees ADL training for facility and staff. Issues and delivers denial notices timely and appropriately. Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, scheduled shift, and, if applicable, differentials related to a specific shift(s)/and or day(s). Responsibilities The Registered Nurse is responsible for the Shift coordination and oversight of the nursing process for residents assigned to his or her care. Responsibilities include both the direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice. Job duties include, but are not limited to: Provide direct nursing care to assigned residents in accordance with the nursing process. May be assigned to function as Medication/Treatment Nurse, unit Charge Nurse, or Supervisor. Direct day to day functions of Licensed Practical Nurses and Certified Nursing Assistants, assuring compliance with state and federal regulations and facility policies and processes. Provide assessment of and responds to resident conditions utilizing the nursing process. Work cooperatively with other members of the facility interdisciplinary team. Maintain effective communication with other members of the interdisciplinary team utilizing facility processes in order to assure the highest standard of care is maintained for each resident assigned to his or her care. Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures. Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services. Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Position Requirements Associate's Degree from credentialed school of nursing.
