Job Details Physician Office l Yellow Springs l First Shift l Full-Time Responsibilities & Requirements Advanced Practice Provider (APP) is a provider of direct primary care health services who synthesizes and utilizes evidence-based practice, research, and current clinical knowledge. Maintain independent clinic schedule of patients with possibility of empanelment. The APP demonstrates an advanced level of medical and/or nursing knowledge, clinical and technical competence, sound clinical judgment, professionalism as it relates to interpersonal and general communication skills, timely and complete documentation and takes responsibility for ongoing professional development and competency validation. Responsibilities & Requirements Under the supervision of the provider and/or practice manager, assists and delivers patient care duties, and provides administrative support throughout the practice. This position assists the provider in examination and treatment of patients, performing miscellaneous delegated duties under the direction of provider. Job Description Your Impact: Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. Job Requirements What You Bring: Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family. State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
