SUMMARY: The P articipant Care Associate (PCA), provides supportive services to the participant; including but not limited to in the home or community setting, during transport and on appointments, or and in the day health center. The PCA’s responsibilities for each participant are specified in the PCA/CNA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The PCA plan of care is based on the initial and on-going assessments that are performed by a home care RNCM. The PCA assists in providing and/or maintaining optimal physical and emotional comforts to frail elders, in the participant’s home, community setting, day health center, and dementia unit, and during transport and on appointments. The PCA assists participants and their families toward achieving maximum self-reliance and independence within their environment, day health center, and dementia unit. The PCA performs the care or services that are outlined in the individual participant’s Care Plan; any Care Guidelines that are pertinent to the participant’s diagnosis are specified in the Care Plan. The PCA performs a variety of tasks and services for the participant, including: Personal Care, Assistance with activities, Assistance with meal preparation/feeding, Assistance with toileting/managing incontinence, simple treatments, shopping, medication reminder, laundry services, extending hours care, homemaking services/chores, transportation and off site appointment support. The PCA is responsible for accurate timely documentation as required by the service area they are supporting; including but not limited to the care and/or services provided on every visit and or encounter, transport and appointment, on the PCA/CNA progress note; the PCA keeps the RNCM and or clinic nurse informed of the participant’s progress. Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor / manager. The PCA responds to emergency situations in accordance with established policies and procedures. Works only within established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES: H.S. diploma or GED required Valid Michigan Driver's license Current CPR card required Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population Experience must be in a long-term care, hospital, or home care setting Must have received certificate of completion from a formal training program (Direct Care, Nurse Assistant, Medical Assistant, Patient Care Technician, etc.) outside of PACE SEMI The PCA participates in annual, mandatory in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training The PCA must possess the ability to establish and maintain interpersonal and interdepartmental relationships The PCA shows respect for the physical, spiritual, and well-being of participants and co-workers The PCA must have the ability to problem solve and interpret instructions The PCA must be able to organize and maintain flexible scheduling to coordinate job responsibilities Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds Ability to relate to those participants with dementia and/or developmental disabilities The PCA must be able to perform duties in an atmosphere of frequent interruption Must meet a standardized set of competencies (approval by CMS) before working independently PCA competency is monitored and maintained in a variety of ways: Initially, through a skills check-off performance evaluation of specific tasks upon hire and orientation to the program, and annually thereafter Job related competency in-services including a practicum demonstration and written assessment Mandatory in-service training Department staff meetings Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. Swan Hospice is actively seeking Registered Nurses (RN) to join our team in Oakland area. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Job Description A. BASIC PURPOSE Oversees primary caseload of Hospice patients and provides skilled professional nursing care to them, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Michigan Hospice licensure laws, accrediting body standards and agency policies and procedures. Provides the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassessment of nursing needs and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. Utilize the nursing body of knowledge to provide leadership and guidance to the IDT in formulating and individualizing the interdisciplinary plan of care to include problem identification, interventions and goals to address palliative comfort measures for the terminally ill patient, family and caregiver. 4. Contribute to identifying both active and potential needs of the patient and his/her family, which may affect the plan of care. 5. Consistently collaborate with the physician to plan and implement the medical and nursing plan of care. 6. Review the nursing component of the comprehensive interdisciplinary plan of care with the IDT. Communicate in writing in the clinical record and verbally with the IDT to coordinate interventions and ensure continuity of care. 7. Facilitate and participate in the IDT care conference and contribute to patient care planning. Implementing and Evaluating Provide nursing interventions and appropriate preventative and therapeutic nursing procedures. Nursing staff shall administer medications in accordance with all Federal and State laws and rules. Collaborates with IDT members to provide continuous assessment of assigned patients, interprets findings, initiates appropriate therapeutic actions and evaluates the results of these actions. Fulfills productivity requirements on an average daily census basis as determined by acuity level and Administrator. Documents accurate, complete and relevant clinical notes regarding patient’s condition and care given. Maintains documentation a on a timely basis. Assesses the dietary/nutritional needs of Hospice patient and provides appropriate interventions relative to nutritional intake as the patient’s condition progresses. Education The RN is a mentor who is able to provide educational experiences and communicate knowledge of patient care to members of the IDT including, but not limited to, volunteers, hospice aide/homemaker, patient and family/caregivers, health care professionals and the general community. Acts as a mentor to other personnel by participating in orientation and assisting with field training and in-service education to contractors and referral sources, as needed. Participates in formal and informal in-services, and patient care conferences to promote professional growth of all interdisciplinary team members. Teaches the patient, family and/or caregiver regarding the progression of the disease process and self-care techniques as appropriate, which includes providing medication, dietary/nutritional counseling support and other instructions as ordered by physician, recognizing and utilizing opportunities for health counseling. Supervises Hospice Aides/Homemakers every fourteen (14) days per Medicare Conditions of Participation. Evaluates effectiveness of educational experiences and teaching methods in order to promote ongoing professional development of Hospice staff and quality patient outcomes for patients, families and caregivers. Case Management The RN is responsible for managing all patients assigned to his/her primary caseload by utilizing the following methods: Efficiency 1. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. Job Summary This position is a 12-week internship program which prepares the new Graduate Nurse for a Registered Nurse position on a general care pediatric unit. Orientation includes a combination of educational sessions and precepted bedside clinical experience. A two (2) year commitment with a minimum of 30 hours/week (0.75 FTR) is required for candidates hired through the internship program. Education will be provided in a group and unit-based settings. Precepted bedside experience will occur on the onboarding unit. This posting will be used to hire two graduate nurse internship positions in the pediatric Hematology/Oncology unit. This internship will have a start date of February, 2027 Flexibility will be available for hours to range from 32-40 hours per week. Brief Descriptions Of The Unit Below 7 East population ranges in age from newborn to young adulthood. The diagnoses range from hematological disorders, such as chronic granulomatous disease and sickle cell anemia, to oncological disorders including leukemia, lymphoma, and solid tumor. 7 East Mott's Bone Marrow Transplant program does 40-50 autologous, cord, allogenic and unrelated transplants a year. 7 East is a 32 bed unit that has all HEPA filtered private rooms and some positive pressure rooms to protect our immunocompromised patients. Expectations Of Position Requirements of the Commission on Cancer Certification must be met within the triannual certification window proportionate to hire or transfer to the oncology area, details provided by leadership. Required Qualifications* The candidate must be a Graduate Nurse or a newly licensed in the State of Michigan as a Registered Nurse. The candidate must not have yet practiced as a Registered Nurse. Successful completion of the NCLEX must occur prior to the start date of the nurse internship. Applicants must be able to start February of 2027. This position requires standing/walking/positioning/pulling/lifting of heavy patients and objects for long periods of time. General Summary The Physician Clinical RN Coordinator provides health care services to patients both in the hospital and clinic setting under collaboration, direction, and responsibility of a physician. Essential Duties and Responsibilities Compiles and documents all medical data on new admits and inpatients to include chief compliant/visit to assist with physician diagnosis. Dictates history, physicals, progress notes, operative notes, and discharge summaries for patient medical record and physician assistance. Tracks patients who are seen in the hospital for coding and billing. Monitors and documents in inpatient/outpatient medical record completely, accurately, and monitors for completion for quality and timely documentation. Educates patient and family members on medical condition, preventative health maintenance, and supportive services to assist with on-going health maintenance. Tracks quality assurance data and monitors for acute and chronic care management to improve patient care processes and outcomes. Administers fluids, medication, treatments, assists with procedures (i.e. surgical, special) and examinations, sets up needed instruments and supplies all as directed by physician. Maintains inventory of medications and supplies, venous access devices (ports), administers IV push medications and flushes as ordered by physician. Performs point of care lab testing, obtains blood samples and other specimens as ordered by physician. Performs daily quality checks for equipment and point of care tests to ensure integrity of testing and quality of equipment. Processes prescription refill requests, referrals for consultative, diagnostic, and therapeutic services by obtaining pre-certification or authorizations to assist patient and insurance requirements. Meets commitments, works independently, accepts accountability, handles change, sets personal standards, stays focused under pressure, and meets attendance/punctuality requirements. Education Graduate of an accredited school of nursing RN program required. Work Experience Minimum three years clinical nursing experience required. Certification/License Current RN license in the state of Missouri or Compact State required. American Heart Association Basic Life Support required.
