JOB SUMMARY: The Pediatric Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education/experience. RESPONSIBILITIES: Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops the plan of care with patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient/family as appropriate per assessment and plan. Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient Knowledge of high-tech nursing skills as applicable which is not limited to indwelling/straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy. Working knowledge of all infection control procedures and safety precautions. Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process. Observe ethical and legal guidelines for safeguarding confidentiality of patients. Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with other health related persons to coordinate care plan. Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary supplies and services are available. Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation. Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days. Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists. QUALIFICATIONS: Bachelor's degree of Nursing from an accredited college/university Current active and unrestricted nursing license Pediatric care experience in the acute and/or home care environment Valid driver’s license Current certification in Basic Life Support for Health Care Professionals. Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist. The Registered Nurse is responsible for the delivery of patient care services through coordination, implementation, ad supervision of field staff. The Registered Nurse Supervisor ensures that field staff follows the POC and delivers care accordingly. The Registered Nurse particopates in quality improvement activities within the agency, promoting overall compliance with State and Federal guidelines and professional standards of practice. Position qualifications: * Must be a registered nurse with a public health, home health care, or hospital background * Must have at least 2 years clinical experience Physical qualifications: * Must be able to hear and speak in a manner understood by most persons. * Must be able to travel to prospective patient's place of residence * Must be able to stoop and bend; must be able to lift and transfer patients. Duties and responsibilities: Demonstrates leadership and provides direction to members of the patient care team through case management of patient home health episode of health care. Case management of patient services includes: A. Initial in home evaluations and/or assessment B. Establish the patient's individual treatment and medical record baseline. C. Collaborate with physicians; other agency staff and contract services to coordinate and implement the POC D. Implement the POC and the quality of care delivered to the patient E. Provide in home supervision of field staff at least every 90 to 180 days days for patient receiving aide services and document the visit in the medical record (EHR) F. Provide supervision of field staff assigned to patient caseload, through directing the implementation and evaluating the effectiveness of the nursing care and patient’s plan of care G. Re-evaluate the patient plan of care at least every 90 to 180 days and when there is a change in the patient condition, after a hospital stay, and a time of discharge revising the Plan of Care as needed to achieve patient goals. H. Conduct new employee orientation and in-services as needed. K. Documents direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy. L. Documentation is timely and meets professional standards of practice, establishes compliance with state regulations and Medicaid conditions of participation. M. Reviews caseload and charts documentation to ensure compliance and assesses patient progress and effectiveness of care delivery in meeting patient goals. N. Ensures that MD orders are faxed timely, follow with MD offices to ensure timely receipt O. Conducts employee physical as needed P. Completes employee annual evaluations Q. F/U calls to clients/families and physicians on all clinical matters R. F/U calls to clients/families and referral sources to respond to complaints S. Collaborating with leadership/management team and office staff to facilitate timely office, processing of paperwork and personnel information T. Actively participates in Agency quality improvement activities U.
