The RN is responsible for admission of patients for home care services, development of the patient plan of care, coordination with patient's provider and other disciplines for the provision of services, and safe delivery of patient care. Collaborates with providers, other disciplines, and communicates all changes promptly. 3. Assesses payment sources and is responsible for ensuring ongoing authorizations as necessary. 4. Educates and instructs patients/caregivers in their ongoing medical maintenance. Begins discharge planning on admission. 5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or other disciplines with supporting documentation. 6. Evaluation of patient progress and prompt action 7. Oversight of LPNs and Home Health Aides. 8. Schedules patients independently according to the individual plan of care. 9. Attends all staff meetings, inservices, other required meetings. Compliance with all mandatory requirements. 10. Follows policies and procedures. 11. Thorough and accurate OASIS documentation. 12. Coordinates and manages the care of patients receiving services in their home. 12.1. Participates in case management conferences and documents per guidelines. 13. May perform other duties as assigned. Qualifications EDUCATION Nursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred. EXPERIENCE Two years nursing experience preferred. Previous home care experience desirable, but not required. LICENSURE Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire. SPECIAL SKILLS Excellent clinical knowledge, verbal/written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team. Overview Registeredl Nurse - Stony Brook Children’s Service, UFPC Location: East Setauket, NY Schedule: Full time - 40 hours - * five 8-hour shifts per week * Days/Hours: Work shifts are scheduled within the hours of 7:30 AM – 8:00 PM. 1-2 evening shifts per week (ending at 8:00 PM) and 1-2 Saturdays per month are required. Schedules are communicated in advance to support planning and work-life balance. Provides supervision to auxiliary personnel assisting with the delivery of patient care. Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Triage patient phone calls and troubleshoot medication management issues. Demonstrates competency in answering phones, assembling charts, patient registration, making appointments, computer data entry and retrieval of reports. Must safely perform all patient care responsibilities in accordance with professional standards and the organization’s policies and procedures. Develop and organize a plan of care in consultation with treating physician, as indicated within the various subspecialties of the Department. Must effectively determine and set priorities to meet patient needs. Provide proper and accurate documentation of patient care in the patient medical record. Coordinate any services or accommodations necessary (e.g. sign language interpreter) to ensure effective communications between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals. Administer medications, treatments, and contrast agents according to the specific orders of the ordering physician, and document in the patient record. Maintains infection control precautions and identifies potential infection control problems. Provides in-service education to members of the interdisciplinary team within the Department. Participate in Quality Assurance activities within the Department. Participates in annual recertification training, compliance training, and HIPAA training, as well as all other mandatory in-service programs. Conforms to policies and regulations governing patient rights and confidentiality of information. Perform all other related duties as assigned by management. Qualifications Required Education & Qualifications: Associate’s degree in Nursing. (New York State BSN in 10 Law - Must attain Bachelor’s in Nursing or higher within 10 years of initial licensure. Current RNs, as well as those currently enrolled or pending acceptance into a program preparing registered nurses effective date of this act (which is Dec. 19, 2017) are grandfathered in.) Current registration to practice as a Licensed Registered Nurse in New York State. Basic Life Support (BLS) required. Must have excellent communication skills. Preferred Qualifications: Bachelor’s degree in Nursing. Advanced Cardiovascular Life Support (ACLS) preferred.
