Located in Chicago’s Streeterville neighborhood, UChicago Medicine River East is a state-of-the-art outpatient center featuring a wide range of care services. In addition to those listed below, we offer infusion therapy, lab services, and radiology services, including CT, MRI, mammography, X-ray, DEXA scan and ultrasound. A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice. This position is responsible for helping guide clinical practice at all clinics and locations across the system, including onboarding of all clinical staff, continuing education, annual competency, ambulatory quality, regulatory compliance, ambulatory policy/protocol development and review, and supporting any new clinical workflows or new implementations. A Clinical Nurse Educator is a registered nurse (RN) with previous educational experience whose focus is on staff development and nursing education. SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions.
