Job description: We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. Key Responsibilities: Provide direct nursing care and clinical oversight for individuals with developmental disabilities in accordance with OPWDD guidelines. Administer prescribed medications and treatments, monitor for side effects, and maintain accurate medical records. Conduct health assessments and respond to medical emergencies as needed. Collaborate with interdisciplinary teams, including DSPs, medical providers, therapists, and family members. Administers medications when appropriate, and supervises AMAP certified staff in medication administration. Supervises all medication prescribed for individuals in accordance with agency policies. Provides supervision and training for Licensed Practical Nurses and/or less experienced. Provides immediate first aid with referral to outside emergency services or private physician. Compensation The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $25.1095 - $33.8557 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
