Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG. Performs other duties/projects as assigned. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State Licensee as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in hospice or home health environment is preferred Nursing skills as defined by accepted nursing standards Ability to handle multiple priorities, documentation requirements, and deadlines Able and willing to travel up to 50% as needed to support business and patient needs. Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. In this role, you will perform a variety of tasks, such as checking supplies and equipment, setting up sterile tables, passing instruments to the surgeon and sterilizing instruments after procedures. This position offers a unique way to gain hands-on experience in an operative environment while making a lasting difference in the lives of patients and caregivers every day. Ultimately, surgery could not be possible without your support. A caregiver in this position works varying days from 6:30AM – 7:00PM or 7:00AM – 3 :30PM, with on-call requirements. A caregiver who excels in this role will: Perform scrub functions and assist in circulating functions under the direction of a Registered Nurse (RN). Set up the sterile table and ensure appropriate supplies , drugs and equipment are available. Assists RN in counting all sponges, instruments, etc. at appropriate times during surgery. Assist in gowning and gloving surgical team members and draping patients. Ensure aseptic condition is maintained by the scrubbed surgical team through the procedure. Participate in the intraoperative “timeout.” Collect, identify and label specimens removed during surgical procedures and gives specimens to circulator for disposition.
