This position would help cover call for nights and weekends. This position is a Full-Time position working 7 days on 7 days off. The working hours for the 7 days on are: Monday-Thursday 5:00pm-8:30am; Friday 5:00pm - Monday 8:30am. Responsibilities: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs for after-hours calls. Provides a complete physical assessment and history of current and previous illness. Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions, and completing week end admissions. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care Re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. After hours on-call responsibilities included remaining available for all calls related to patient care and making all appropriate visits as necessary. Remaining with patient and or family through a crisis period until resolved and or relived. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for the resident. Prepares residents for meals, serves and removes food trays and assists with meals or feeds residents, as necessary. Monitors nutritional intake. Distributes drinking water and other nourishments to residents. Monitors intake. Knowledgeable about the types of isolation, performs general care activities for residents in isolation. Answers residents' call lights, anticipates residents' needs and makes rounds to assigned residents. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices). Transports residents to and from various departments as requested. Reports and, when appropriate, records any changes observed in condition or behavior of residents and unusual incidents. Completes STOP & WATCH. Participates in and contributes to interdisciplinary care conferences. Must be able to follow directions, both oral and written, and work cooperatively with other staff members. Must have the ability to acquire knowledge of and develop skills in basic nursing procedures and simple documenting. Establishes and maintains interpersonal relationship with residents, family members, and other facility staff while assuring confidentiality of resident information. Attends in service education programs, as assigned, to learn new treatments, procedures, developmental skills, etc. Practices careful, efficient and non wasteful use of supplies and linen and follows established charge procedure for resident charge items. Maintains personal health in order to prevent absence from work due to health problems. Possesses a genuine interest and concern for geriatric and disabled persons. Essential Job Functions: • Depending on location, performs phlebotomy and specimen processing as directed by delegating provider. • Administers, including but not limited to, intramuscular injections, subcutaneous injections, intradermal skin testing, topical medications, ophthalmic drops and nasal spray. • Depending on location, performs ECGs, places Holter monitors and downloads telemetry data. • Works in conjunction with other staff to maximize clinic flow. • Intervenes appropriately in emergency situations. • Documents data and changes in the Electronic Medical Record. • Provides assistance and or back up support for administrative tasks including but not limited to scheduling patient visits and performing basic “check-in, check out” activities. • Obtains measurements, patient history and other intake activities. • Stocks and maintains the clinic area including ordering of supplies as needed. • Demonstrates knowledge of pertinent aspects and competency with protocols related to the use of chemical disinfection, high level disinfection, and sterilization if required as part of the responsibilities of the role. • Depending on location, directs High Level Disinfection scope cleaning in accordance with defined HLD processes. • Completes trainings and education including but not limited to Healthcare Provider CPR. • Other job functions as assigned. Knowledge, Skills, and Abilities: • High school diploma required. • Successful completion of a formal Medical Assistant program. • Possesses 0 – 3 years of relevant clinical experience. • Certified Medical Assistant (CMA) (AAMA) or Registered Medical Assistant (RMA) preferred but not required. • Some college, preferred. • Basic Life Support certification from the American Heart Association. • Excellent communication/listening skills.
