Seeking a certified, caring and compassionate GNA to provide routine daily nursing care (ADL's) and services that support the care delivered to residents residing in the facility in accordance with the established nursing care policy and procedures and as directed by the supervisor. Contributes to the provision of high-quality, cost-effective healthcare as a provider of direct and indirect patient care and by effectively collaborating with other members of the health care team. The LPN contributes to the nursing process in a team relationship with the Registered Nurse. Efficiently implements the patient's plan of care in accordance with applicable standards, policies, procedures, and guidelines. Demonstrates proficiency in medication administration, pain management, and other unit or initiative-specific skills. Organizes and reprioritizes patient care activities based on patient and/or environmental changes. Acts as a patient 1:1 sitter as needed. Continuously evaluates the effectiveness of the plan(s) of care, making recommendations for revision to the health care team based on analysis of patient responses to interventions. Consistently communicates changes in patient status to the appropriate members of the health care team. Participates in QI, CPI, and risk management activities at the unit, department, or organizational level. Participates in unit shared governance according to departmental standards. Participates in the education and orientation of new staff; evaluates delegated activities for expected patient care outcomes. Educational/Experience Requirements: Successful completion of the LPN IV Medication Course and the NLM Medication Exam. HS diploma/equivalent Current enrollment in LPN to BSN Bridge Program Two years Nursing Rehab and/or Skilled Nursing experience Long Term Acute Care Hospital experience (LTACH) NRP (Nurse Residency Program) is preferred within 1 year of employment Required License/Certifications: Adherence to credentialing requirements of Luminis Health as stated in the nursing bylaws. The Hospice Admissions Nurse reports to the Admissions Team Manager. The Hospice Admissions Nurse assesses patients in their place of residence or in hospitals and prepares the patient for entry into the spectrum of services provided by Gilchrist Hospice Care. The Hospice Admissions Nurse serves also as ambassador for Gilchrist Hospice Care within the service area. Education: Diploma, ADN and/or BSN from an accredited institution Licensures/Certifications: Current MD nursing license. Experience: Three years of nursing. Home Health or Hospice experience preferred but not required. Skills: • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection • Strong time management skills and the ability to function with a changing daily schedule. • Excellent interpersonal and communication skills • Ability to be patient and family focused and provide patient/caregiver education • Skill in the use of EMR (Electronic Medical Record) Principal Duties and Responsibilities: • Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs - including physical, emotional, spiritual and mental status. • Prepares and implements the initial care plan for patient/family unit with Medical Director approval. • Provides direct nursing care to patients in order to maintain highest level of care. • Inform Team Leader of utilization of other team members, outside agencies, and community resources upon initial assessment findings. • Document clinical findings to assure initial hospice eligibility and participate in discharge planning when necessary working with relevant patient care, social services, and other departments. • Teaches family members or hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician • Ensures that arrangements for equipment necessary in the home setting are made available. • Attends to patient and family at time of patient’s death if it occurs at the time of visit. • Participates in Admissions department projects and process improvement strategies. • Establishes and maintains effective working relationships with all possible Hospice referral sources in the service area. The RN Care Manager is committed to comprehensive quality care management and coordination of clinical activities for primary care patients. This position focuses on promoting early intervention for complex patients and co-developing a plan of care that prevents unnecessary complications and negative patient outcomes. The RN Care Manager promotes population management through effective education, self-management support, goal setting, and timely health care delivery. This includes communicating with patients and their families concerning the progress of patient goals and ongoing care needs, developing and monitoring care management processes, and support of primary clinical team efforts. The Nurse Care Manager develops and maintains collaborative working relationships with all team members including Practice Manager, Lead Physician, and Care Coordinator, and Behavioral Health Care Manager to best serve the needs of the identified patient panel and primary care teams. The RN Care Manager will serve as a clinical resource to the Medical Assistant staff and Practice Manager. Education: Registered Nurse license required. Bachelor’s of Science in Nursing preferred. The responsibilities of this job include, but are not limited to, the following: Supervising the medical care of all patients from registration to departure; Supervising clinical personnel on their patient contact, care, and performance; Scheduling Staff Nurses and Medical Assistants; Reviewing and supervising the inventory of medicines, prescriptions, and supplies; Acting as Patient First representative for the Director of Medical Support and the Medical Director in his or her absence to ensure smooth operation of the center; Functioning as an administrator and working as a Staff Nurse as needed; Reviewing all checklists of Staff Nurses and Medical Assistants; Preparing written and verbal evaluations for Staff Nurses and Medical Assistants at appropriate scheduled time, or when necessary; Providing positive, warm and friendly customer service in all interactions; Fostering teamwork and a positive, professional atmosphere; Completing other duties as directed. Minimum education and professional requirements include, but are not limited to, the following: Employee must be at least 18 years of age; High school graduate or equivalent; Keyboarding experience required; Excellent verbal and written communication skills; One year of clerical experience preferred; One year of clinical experience with administration of medications preferred; One year of supervisory experience preferred; Licensed to practice as a Nurse in the state of Maryland. Salary Range: $42.50 - $47.50, depending on experience.
