Job Description Nemours Children’s Health is seeking an experienced pediatric telephone triage nurse to support our primary care practices in Delaware, Pennsylvania and Florida. This position is FULL-TIME, 72 hours biweekly, 12-hr rotating shifts, with every third weekend and holiday rotation . 3 East is a surgical intermediate unit with a high population of patients requiring post-operative care for urological issues, traumatic injuries, and general surgery. 3 East also cares for patients with acute and end stage renal disease and patients requiring an intermediate level of nursing care due to their tracheostomy and/or ventilator needs. Each patient room is equipped with oxygen, compressed air, and suction along with state-of-the-art cardiorespiratory bedside monitors and computers that enable the healthcare team to access a patient’s electronic medical record. The patient care complexity directly relates to the wide range of patient care skills required for this diverse patient population. The goal of this unit is to provide high-quality, cost-effective patient care. The Synergy model is used in making appropriate patient assignments based on patient and family needs and nursing skill set. Special equipment used on this unit includes infusion pumps, syringe pumps, feeding pumps, BiPap respirators, Oxygen delivery methods, PCA & epidural pumps, wound vacutainers, ventilators and specialty beds. Nursing competencies include: chest tube management, pain management, blood product administration, urinary catheterization, tracheostomy care, ventilator management, and central line care and management. Job Details Christiana Care is currently seeking a part-time hybrid advanced practice provider (PA/NP) for the Christiana Care ED callback team. In this role you will work collaboratively within the service line and with other departments to ensure timely follow-up of ED results. The APP will utilize established protocols to determine the need for further evaluation, testing, or treatment and will communicat e this information directly to patient and /or other healthcare providers. This is a part-time day shift position covering 40 hours biweekly with flexibility to allow for some of these hours to be done virtually. The selected candidate will work alongside other callback providers to provide coverage 7 days a week including holidays. This role oversees results for CCHS’ Delaware and Maryland campuses. Principal Duties and Responsibilities : Review all results (lab and radiology) for patients who are discharged from a CCHS Emergency Department . Contact patients to inform of pertinent post-discharge results per callback guidelines and p erform necessary follow-up actions which include , but are not limited to , prescribing medications, coordinating outpatient follow-up care, and patient education Receive and manage incoming calls from patients and pharmacist s to assist with visit related questions and prescription questions/medication adjustments. Return voicemail message s received during off hours .
