Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as prescribed Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals’ responses to medication or changes in health status. Serves as the nursing representative on a treatment team; provides information on nursing needs; and consults and coordinates with treatment teams and other health care disciplines to assess, plan, implement, or evaluate individuals’ care. Makes clinical rounds to assess individuals’ nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs. Implements infection control protocols and procedures and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. POSITION SPECIFIC DUTIES may include but are not limited to: Provide nursing services to assigned caseloads; This RN will provide nursing oversight to approximately 20 developmentally disabled individuals who are medically and/or have increased psychiatric needs. This RN will also provide the OPWDD required RN oversight for approximately 40 unlicensed staff regarding individual- specific nursing procedures such as completing assessments, conducting weekly site visits for assigned caseload of homes, nursing triage to unlicensed staff and individuals, clinical supervision of assigned tasks to unlicensed staff, development of plans of nursing services, monitoring of staff’s completion of directives, Staff medication certification/re-certifications, and in-servicing, Medication error tracking and remediation and Medication audits. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. Geographic Pay Differential is added to the job rate ($88,970). Demonstrates ability to adjust to changes in units/shift assignments to meet resident and facility needs. 7. Demonstrates knowledge and ability to provide care on specialized dementia unit, as appropriate. 8. Successful completion of facility orientation on the care of a resident on a specialized dementia unit, as appropriate. 9. Demonstrates knowledge of and complies with the Code of Conduct and Corporate Compliance Program when performing work functions. B. Resident Care Responsibilities: 1. Answers call bells on a timely basis to respond to resident needs. 2. Completes or assists residents with bathing, dressing, hygiene, and grooming, in accordance with established Care Plan. 3. Assists residents with oral care (teeth brushing) as per Care Plan. 4. Weighs residents and records weights monthly/weekly or more frequently as assigned. 5. Serves and collects food trays, feeds residents requiring help, provides meal supplements and fresh drinking water as needed. 6. Moves or assists residents in moving to and from bed as necessary, and transports residents using wheelchairs, or assists them with walking, as per Care Plan. 7. Takes vital signs on all residents as per Care Plan including temperature, pulse, respiration and blood pressure. Notifies Charge Nurse of any changes or findings beyond normal limits. 8. Records food and liquid intake and emission as directed. 9. Reports all unusual conditions and reactions to nurse in charge. 10. Assists in keeping residents’ rooms and equipment clean, neat and orderly. Cleans and maintains therapeutic equipment (i.e. nebulizers, oxygen concentrators, IV poles and pumps, wheelchairs, gerichairs, siderail pads, linen carts, med carts, treatment carts, tub rooms, etc.). Dates and labels all personal hygiene items. Removes and cleans all therapeutic equipment and returns to its appropriate storage area if a resident is discharged or expires. 11. Changes bed linen and incontinent care items as needed. 12. Follows established systems: a) Releases restraints and repositions residents according to established Care Plan. b) Ensures that all residents are out of bed according to established Care Plan (if appropriate). If a resident does not get out of bed, notifies Charge Nurse. c) Turns, positions and toilets residents as per Care Plan. d) Ambulates residents according to the plan of care and facility policy. C. Role Responsibilities-Interpersonal Skills: 1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information. 2. Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner. 3. Addresses resident and family satisfaction issues immediately by reporting them to the Charge Nurse in a professional manner. 4. Is aware of Resident Abuse Reporting Law. 5. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and enthusiastic attitude. D. Role Responsibilities-Safety: 1. Demonstrates ability to locate emergency equipment (i.e. emergency oxygen tank). 2. Understands and can demonstrate knowledge of role in Fire and Disaster drills. 3. Demonstrates ability to identify/respond appropriately to potential behavioral outbursts. 4. Uses facility equipment safely. 5. Recognizes, removes, and/or reports potential hazards. E. Resident Care Responsibilities-Infection Control: 1. Demonstrates knowledge of and implements universal precautions. 2. Washes hands between resident contacts as appropriate. 3. Handles linen and disposes of waste properly. 4. Demonstrates ability to care for residents on precautions. 5. Demonstrates knowledge of correct positioning/care of catheters and drainage. F. Resident Care Responsibilities-Resident Dignity: 1. Addresses residents in a respectful manner. 2. Discusses confidential resident information in appropriate areas only. 3. Knocks on door prior to entering resident rooms, and maintains resident privacy and dignity when providing personal care such as bathing, dressing and toileting. G. Other Duties as Assigned by Charge Nurse or Resident Care Coordinator. SUMMARY OF QUALIFICATIONS 1. High School Degree and previous Long Term Care Nursing Assistant experience preferred. 2. Must be at least 18 years of age. 3. Successfully completes facility conducted orientation, mandatory training and in-service programs. 3. Meets health assessment requirements, including two stage Mantoux skin test. 4. Is listed on the New York State Nurse Aide Registry. 5. Possesses a current (not expired or lapsed) New York State Nurse Aide Certification. 6. Has no findings of resident abuse, mistreatment, neglect, or misappropriation of resident property listed on the registry. 7. Position: School Nurse - Substitute Registered Nurse (approximately October 26, 2026 to December 23, 2026) Performance and Responsibilities : Develops policies, procedures and work standards for school heath program in collaboration with other district school nurse staff. Monitors compliance of school health program with federal, state and local laws, regulations and policies. Manage program allotment efficiently. Resident Care Responsibilities: 1) Assumes responsibility on assigned unit for: Medication & Treatments a) Administers enteral feedings, and oral, injectable and intravenous medications and treatments within the LPN scope of practice as prescribed by the Physician and per facility policy. b) Performs and is co-responsible for narcotic count as per Resident Care Policies and Procedures. c) Demonstrates expertise in dispensing medications and treatments by all routes as ordered by the Physician with established time frames. Ensures no medications are left unattended. d) Assesses effectiveness and observes for interaction and reaction of medications and treatment. e) Demonstrates knowledge of residents’ allergies according to established Care Plan. f) Documents medication and treatment administration in a timely and accurate manner according to Resident Care Policies and Procedures. g) Orders appropriate medications from pharmacy as needed in accordance with Resident Care Policies and Procedures. Management and Supervision of Nursing Assistants: a) Reports pertinent information to Nursing Assistants on assigned residents prior to delivery of care, and provides direction in prioritizing resident care. b) Reports resident information to Nurses and Nursing Assistants reporting for next shift. c) Ensures all aspects of delivery of care to residents is completed within the established time frames according to Resident Care Policies and Procedures. d) Ensures all aspects of resident care information is up-to-date and reviewed by Nursing Assistants prior to the delivery of care. 2) Complies with all State Department codes and requirements regarding nursing care and documentation. C. Role Responsibilities-Interpersonal Skills: 1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information. 2. Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner. 3. Addresses resident and family satisfaction issues immediately by reporting them to the Supervisor in a professional manner. 4. Is aware of Resident Abuse Reporting Law. 5. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and enthusiastic attitude. D. Role Responsibilities-Safety: 1. Demonstrates ability to locate emergency equipment (i.e., emergency oxygen tank, etc.) 2. Understands and demonstrates knowledge of role in Fire and Disaster drills. 3. Role Responsibilities-Infection Control: 1. Demonstrates knowledge of and implements universal precautions. 2. Washes hands between resident contacts as appropriate. 3. Handles linen and disposes of waste properly. 4. Demonstrates ability to care for residents on precautions. 5. Demonstrates knowledge of correct positioning/care of catheters and drainage. F. Role Responsibilities-Resident Dignity: 1. Addresses residents in a respectful manner. 2. Discusses confidential resident information in appropriate areas only. 3. Knocks on door prior to entering resident rooms, and maintains resident privacy & dignity when providing treatments or other resident care. G. Other Duties as Assigned by Shift Supervisor, Head Nurse and/or Resident Care Coordinator. SUMMARY OF SUMMARY OF QUALIFICATIONS 1. Current license as a Licensed Practical Nurse in the State of New York required. 2. Is a graduate of an accredited school of Licensed Practical Nursing. 3. Successfully completes facility conducted orientation, mandatory training and in-service programs. 4. Previous long term care experience is required. Supervisory experience and organizational skills in a long term care setting preferred. 5. Must meet health assessment requirements, including two stage Mantoux skin test. 6. Is listed in good standing as confirmed on the New York State Department of Professional Credentialing. 7. Possesses excellent technical assessment and documentation skills, and leadership quantities. 8. Must be capable of performing the essential functions of the job, with or without reasonable accommodations. 9. Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
