We are seeking a Registered Nurse (RN) for their Crisis Respite/Transitional Program (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The Registered Nurse (RN) provides comprehensive nursing services across the CRTP, ICM, and CTI. This full-time role supports individuals with dual diagnoses of intellectual and developmental disabilities (IDD) and severe mental illness (SMI) needs by integrating skilled nursing care, crisis stabilization, transitional planning, and intensive care coordination. The RN collaborates with interdisciplinary teams to promote health, safety, dignity, and successful community integration. The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Salary : $95,000 per year plus benefits. Schedule: Monday - Friday 9am-5pm Essential Responsibilities (including but are not limited to): Clinical Care -Screen individuals who are being considered for admission to the CRTP and the Transitional Respite Program. Active member of the "intake” team. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. -Oversight of self-medication administration program, monitor therapeutic response, and document outcomes. -Provide direct nursing interventions such as wound care, diabetes management, respiratory illness support, and chronic disease oversight. -Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions. -Regularly reviews medication administration records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. -Reports significant issues to appropriate clinicians and team members, including the need for ordering medications, renewing or updating prescription orders, and people-supported specific drug information sheets. -Monitor and manage medication storage and disposal procedures in accordance with federal and state law. -Develop and monitor health goals that are person specific. Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Responsibilities Patient Care Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. Leads staff participation in quality and safety initiatives and mandated compliance measures. Operations and administration Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. Works collaboratively with physician dyads and the healthcare team to promote patient care and regulatory standards. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. Oversees 24-hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. Maintains control of established positions and manages the recruitment and retention of qualified staff. Maintains patient/employee confidentiality in the management of information. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. Monitors availability and maintenance of nursing equipment and supplies. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. Facilitates organizational change initiatives by using effective strategies. Leads a shared decision-making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Periodically review the department’s policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Issue verbal and written disciplinary warnings to assigned CNAs/GNAs and other nursing personnel for violations of current rules, regulations, and guidelines of the Center. Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained. Ensure that all nursing service personnel are in compliance with their respective job descriptions. Participate in the maintenance and implementation of the Center’s quality assurance program for the nursing service department. Participate in Center surveys or inspections made by authorized government agencies, as may be requested. Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required. Assist in planning the nursing services portion of the resident’s discharge plan, as necessary. Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies, as required. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Education Must possess, at a minimum, a Nursing Degree from an accredited college or university, or graduate from an approved LPN/RN program. Experience Not applicable. On-the-job training provided.
