Job Summary JOB DESCRIPTION Under the supervision of the UCSF Primary Care Administrative Nurse Manager, guided by policies and procedures of the Ambulatory Services, the Hospital Assistant assists in the delivery of care to patients at UCSF Primary Care. The Hospital Assistant will be primarily responsible for the members of their primary care team as well as providing back up support to other teammates. The Hospital Assistant will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. This position floats between the front and the back office performing both clerical and clinical duties. Work includes a variety of personal care and nursing care procedures including assuring positive care outcomes. S/he will perform assigned patient care and laboratory procedures in support of services provided to patients within the practices. Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. This position is primarily night-based and offers the opportunity to practice at the forefront of advanced heart failure, transplant, and MCS care within a leading academic medical center. Responsibilities 60% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1.Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. The Cardiothoracic Surgery Advanced Health Practitioner position has clinical, procedural, administrative, and educational responsibilities in the care of adult patients requiring cardiac and thoracic surgery. The CT surgery service provides advanced surgical treatments, including coronary artery bypass grafting, aortic and mitral valve replacement, extracorporeal membrane oxygenation, and heart transplantation. Job Description Under the direction of the Ward 86 Nurse Manager, the Registered Nurse (RN) will work in a primary care setting, delivering comprehensive nursing care within a multi-disciplinary team structure. Ward 86 is the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN) and also provide preventive services to patients at-risk for HIV infection, including pre-exposure prophylaxis (PrEP). Ward 86 provides care to approximately 2500 patients living with HIV, with over 24,000 clinical encounters a year.
