Job Description We are seeking an experienced nurse educator to join our Advanced Heart Failure Comprehensive Care Center team. In this role, you will play a vital part in educating and empowering patients and staff, ensuring they have the knowledge and skills to navigate advanced heart failure therapies and post-transplant care. Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. The APP has responsibility for the quality of standards of care for a specified patient population and evidences self-direction and accountability in the development of this role within the all UCSF medical center locations. UCSF Health requires all Advanced Practice Providers (APP) to be credentialed through its Medical Staff Services Office (MSSO) in order to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings at all UCSF Health sites (Parnassus, Mt. Zion, Satellite Clinics, et al). Job Description UCSF nurse practitioners (NPs) are advanced practice providers (APPs) who possess the education, national board certification, California licensure, knowledge, clinical skills, and experience necessary to provide evidence-based patient care in their area of clinical practice. They provide routine, acute, critical, and specialty care as defined by their granted privileges/standardized procedures. These may include performing non-invasive or invasive procedures and operative first assist. The NPII will work as part of a Team at the Parnassus location within the UCSF Department of Radiation Oncology in a multidisciplinary environment working with Radiation Oncologists, Residents, Registered Nurses, Medical Physicists, Radiation Therapists, Dieticians, and Social Workers in collaboration with referring medical oncologists and surgeons. The NPII reports to and is managed by an APP Manager while working under the supervision of Faculty/Attending Providers. The patient populations may vary but will focus primarily on Solid Tumor cancers. The incumbent will provide care to patients including taking histories; conducting physical exams; document findings and actions as indicated, complex wound care within the department and assist with planning of the course of patients’ treatments for adult populations. The NPII will perform site-specific patient teaching; prepare patients for simulation; may assist with contouring structures for treatment planning; medicate patients as needed; order medications for symptom relief; assist in managing patients in follow-up; and address quality of life issues not limited to skin/wound care, speech/swallowing, adequate nutritional intake; and psycho-social issues. Job Description Under the supervision of the UCSF Department of OB/GYN, REI guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique ambulatory patient services this Nurse Practitioner (NP) position is assigned to in the Center for Reproductive Health (CRH). Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. This position is primarily night-based and offers the opportunity to practice at the forefront of advanced heart failure, transplant, and MCS care within a leading academic medical center. Responsibilities 60% Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 1.1.Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Job Summary JOB DESCRIPTION Under supervision of the Nurse Manager, MA Supervisor, and Charge Nurses, guided by policies and procedures of the Ambulatory Services, the Medical Assistant assists in the delivery of care to patients being seen in the practice. They will perform assigned patient care procedures in support of the care services provided to patients within the practice. They are required to adhere to the Telephone and House Standards as adopted by the department.
