Description Registered Nurse-Station 6 IMCU-Full Time-Nights 7p-7:30a-Mount Sinai Queens The Clinical Nurse is a Registered Professional Nurse who provides safe, competent, quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Description The Transplant Coordinator is a Registered Professional Nurse with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute. Responsibilities Responsible For Development, implementation, coordination and evaluation of clinical programs and services for designated patient populations. Principal Duties and Responsibilities Clinical Practice 1. Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites. The multidisciplinary team includes the transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance. Coordinators work with their assistants to facilitate other medical specialty visits as needed. 2. Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required (including bloodwork sent to Rogosin for tissue typing). 3. Description The Clinical Program Manager provides clinical support and administrative direction within the Tisch Cancer Center’s Clinical Trials Office (TCC CCTO). In collaboration with the Associate Director for Research, clinical trials investigators, and TCC CCTO Administrative Directors; this candidate is responsible for the implementation and conduct of clinical trials with investigational anti-cancer agents in the Cancer Institute. The Candidate must have expert knowledge of cancer as a disease process, cancer treatment modalities, and the process of conducting clinical research. Description The Clinical Program Manager is responsible for the implementation and conduct of clinical trials with investigational anti-cancer agents in the Cancer Institute. The Candidate must have expert knowledge of cancer as a disease process, cancer treatment modalities, and the process of conducting clinical research. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Registered Nurse-Intensive Mobile Team-Full Time Salary: $107,120 plus benefits Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The Registered Nurse (RN) functions as a primary practitioner who assesses and monitors IMT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assists individuals in their achievement of mental health, rehabilitation and recovery goals. Core Principles The job responsibilities of all staff extend to understanding and incorporating certain principles into their work and into their relationships with program participants. Conduct psychiatric, physical health and other assessments with a special focus on conducting assessments and evaluations with individuals using substances or experiencing street homelessness. Develop, or consult and coordinate with team to develop, plans to address participant health needs. Monitor and modify treatment plans as indicated by participant responses and conditions. Use online databases and portals e.g. PSYCKES, Healthix, MAPP, and others to support assessment and treatment planning. Order, interpret and evaluate medical diagnostic tests to identify and assess participant conditions. Provide management and administration of medication in conjunction with the psychiatrist. Provide a range of treatment, rehabilitation, and support services. Utilize motivational interviewing techniques and a trauma informed approach when delivering individual and group based health services. Administer IMs and vaccines, plants and reads PPD's, and performs phlebotomy as per orders Monitor vital signs and side effects of medications and reports findings to team, and within annual health assessments. Collaborate with psychiatrist and other medical personnel to schedule appointments and coordinate care. Make referrals to community physicians and work jointly with hospital and institutional staff. Intervene and advocate on participant's behalf with, for example, collaterals, agencies, facilities and family members; assist with problem solving to help participant access needed supports. Provide consultation to IMT team regarding participant medical issues; serves as a resource to the team on medical concerns. Maintains accurate, detailed reports and records including required documentation, progress notes and health related data, in AWARDS. Evaluate and establish system for reporting on nursing intervention and expected outcomes. Attend and participate in supervision, meetings and training sessions as required. Provide 24-hour crisis intervention services on rotating basis. Job Qualifications Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision. Currently licensed as a registered professional nurse by the New York State Education Department. Minimum two (2) years' experience working as an RN. Minimum two (2) years' experience working with persons with mental health condition in health or social services setting. Excellent written, verbal and interpersonal communication skills. Ability to synthesize and summarize information and make judgments regarding care. Eligible for full and unconditional participation in the Medicaid and Medicare programs. Computer proficiency in Windows operating systems and programs, such as MS Word, Excel, PowerPoint, as well as comfort with learning new electronic systems.
