Advanced Practice Provider - (Nurse Practitioner or Physician Assistant)

Overview The Department of Medicine has developed a Comprehensive Sickle Cell Program in collaboration with the Departments of Physical Medicine and Rehabilitation and Psychiatry. This program is supported by EVMS and Sentara Health. The goal of this program is to provide top-notch outpatient and inpatient clinical care to patients with sickle cell disease. We are looking for Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider in the Division of General Internal Medicine, Department of Medicine. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant and educator as indicated. The clinical practice will mostly focus on clinical care of patients with Sickle Cell Disease in the hospital and in the outpatient clinic under supervision of clinical faculty. The clinical practice may also include provision of primary care to all types of patients. Responsibilities Independently assess, diagnose, & develop treatment plans for patients with sickle cell disease as well as patients in the area of Primary Care in accordance with protocols and standards of care. Promote a schedule of health promotion examinations and interventions in our patient population Obtain medical histories, perform physical examinations, and prescribe, dispense or deliver medications under a collaborative practice agreement. Differentiate between normal and abnormal findings and orders appropriate diagnostic studies. Provide anticipatory guidance and counseling to patients/families. Document patient assessment and plan of care in the medical record and generate appropriate outgoing correspondence. Be familiar with documentation requirements to insure proper billing. Initiate and coordinate referrals to other health care team members or agencies. Serve as a consultative resource to nursing staff. Participate in the development, utilization and evaluation of resources to meet patient/family educational needs. Provide education to external agencies, community and other health professionals. Participate in development and implementation of divisional and departmental quality assessment/improvement/research programs and processes. Monitor task module of the electronic health record to insure timely response. Responsible for administrative activities and special projects as assigned. Contribute toward the achievement of divisional and departmental goals. Participate and contribute to activities that enhance clinical expertise and nursing knowledge. Nurse Practitioner II

Overview Responsible for providing prenatal, postnatal, general medical care and treatment of patients within the Maternal Fetal Medicine division which is a division of the Department of Obstetrics and Gynecology at Eastern Virginia Medical School. Nurse Practitioners are expected to function as an independent provider primarily coordinating pre-pregnancy, prenatal and post-partum care in women with high-risk pregnancies. This position will provide a combination of inpatient and outpatient care duties. Maternal Fetal Medicine nurse practitioners’ function in an expanded nursing role and perform selected tasks and services within the province of medicine as well as tasks that fall within the province of nursing. Nursing and medicine must maintain ongoing collaboration in education and practice so that the nurse practitioner is maximally effective in meeting the primary health-care needs of women. When performing nursing aspects of this role, the practitioner must have the opportunity to engage in independent as well as co-operative decision-making. When performing expanded aspects of the role that overlap with medicine, the practitioner must function in collaboration with qualified physicians within the framework of jointly approved guidelines. Nurse practitioners not only must be prepared and competent to apply additional knowledge and skills, but also must be ready and willing to accept responsibility and accountability for their practice. Overview This is a full time position for a Medical Assistant in the department of Obstetrics & Gynecology, responsible for front and back office as well as assisting Physicians, Nurse Practitioners and Nursing staff with examination and performing Urodynamic Testing of patients. Responsibilities Perform Urodynamic Testing on patients. Skilled at placing urethral catheters. Overview For a professional Registered Nurse to provide outpatient nursing care and assistance to our providers in the Endocrine Clinic and the Strelitz Diabetes Center. The patient population include diabetes and its comorbidities (hypertension, dyslipidemia, obesity etc) as well as thyroid, osteoporosis, pituitary, adrenal, and other hormonal disorders. Must be able to independently perform clinical and administrative duties. Coordinate patient care by; reporting test results, assessment of patient status, educational needs, patient education and follow-up care. Overview The purpose of this position is to coordinate and support the medical needs of patients pursuing Pediatric Otolaryngology service. The position will partner with faculty physicians and residents in Otolaryngology to provide excellent clinical care and customer service for patients with Otolaryngology disorders. This position interacts with other care providers to deliver appropriate, timely care and patient education. In addition, the position will establish and maintain systems and relationships to ensure a smoother care transition from the clinic setting to outside care providers. The position will work in conjunction the Divisions faculty providers and nurse manager to continuously improve coordination of patient care and administrative systems. The position will be active in patient care through Pediatric Otolaryngology offices operated by the Childrens Hospital of the Kings Daughters with office locations in Norfolk, Virginia Beach, Chesapeake and Newport News. Responsibilities Provide outpatient care and medical management as directed for a variety of Pediatric Otolaryngology diagnosis. Orient residents and nursing staff to departmental treatment guidelines and protocols and monitor compliance in an effort to decrease practitioner variability, decrease cost, improve quality and patient satisfaction. Provide initial and follow-up continuity evaluation and management services to patients as assigned; provide initial consultation services; provide continuity follow-up services. Overview Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider in the Division of Geriatrics/Palliative Care/NeuroHealth, Department of Medicine. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant and educator as indicated. The clinical practice will focus on provision of care to all types of patients.
