In this role the Certified Nursing Assistant will: Reports directly to the nurse manager, may also take direction from licensed nurses on duty or other supervisory staff. Position is service in nature, in performance of duties the resident care assistant is entrusted to assist residents with basic personal care according to procedures and daily assignment. This includes but is not limited to bathing, dressing, dental hygiene, toilet activities, grooming, feeding, transportation/transfer, etc. Maintain resident rooms in a clean and orderly manner by providing light housekeeping, changing linen, making beds, putting away clothing and personal effects, tidying bedside table, shelves, closets, and cleaning bathrooms, etc. Assist with resident laundry assuring clothes are marked, clean and mended, ironed, and put away. Notify supervisor of clothing needs as they occur. Prepare residents for meals, assist with serving meals and feeding of residents. May be cross trained in Assisted Living. The ideal candidate will have: High School diploma or the equivalent. Completion and passing the test for an approved Nursing Assistant program in the State. JOB SUMMARY The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility. Assist in oversight of the Memory Care Aides and Certified Nursing Assistants (CNAs), including supervision and assistance with ADL care, transportation, and non-scheduled activities by staff. Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail. Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication. Assist in providing a therapeutic, person-centered memory care activity program that constantly strives to enhance the quality of life of each resident, meets the Memory Care Program Mission and Philosophy, and upholds the Memory Care policies and procedures and all state and federal standards; commit in providing non-scheduled activities through the use of my talents and training. Encourage family involvement through volunteering and visitation. Provide a clean, safe, non-hazardous unit for the residents and staff. Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented. Complete all required documentation. Maintain all program policies and procedures and state and federal regulations. Provide feedback to the Memory Care Director for monitoring achievement of the facility and Memory Care Program goals through a team approach. Be flexible to meet the change in residents’ needs. Recommends to the Director of Nursing the equipment and supply needs of the department. Inventories and stores the new customer’s personal possessions. Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Maintains intake and output records as instructed. Checks and reports bowel movements and character of stools. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Answers call lights promptly, answer telephone calls as needed and run errands as requested. Obtains blood pressure, temperature, pulse, respiration, heights, and weights as needed. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses’ call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately. Registered Nurse (RN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations Provides and receives nursing reports upon reporting in and ending shift duty hours. Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner. Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. Prepares and administers medications and performs treatments as ordered by the physician. Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Counts all controlled drugs with other nurse at shift change and notifies management of all drug and narcotic discrepancies noted on your shift. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Admits, transfers and/or discharges residents as necessary to meet the resident’s individual needs and maintains the overall safety of other residents, staff and visitors.
