About the Role We are looking for a dedicated Registered Nurse (RN) – After Hours to join our hospice team full-time. This position travels throughout the whole Blue Ridge Care territory How You’ll Make a Difference • Answer emergent calls from patients and families. • Provide after-hours visits to address immediate concerns or changes in condition. • Deliver skilled nursing care based on patient goals and the physician’s plan of care. • Assess and evaluate patient status; give guidance and oversight when LPN support is involved. • Develop and update care plans based on nursing assessments. • Administer medications and treatments as prescribed. • Support preventive, restorative, and rehabilitative care needs. • Educate patients and families on symptom management and care techniques. • Offer support and reassurance during challenging and emotional moments. • Complete timely, accurate clinical documentation. • Notify physicians of changes in condition and obtain/document orders. • Ensure the home environment is safe and appropriate for care. • Coordinate needed equipment and services. • Conduct initial assessments and support smooth transitions across the care team. • Collaborate with families and the interdisciplinary team to align care with the patient’s goals. • Participate in continuing education and quality activities. • Support programs such as peer chart reviews, medication reconciliation, and tuck-in calls. Provides primary care in collaborative joint practice with the physicians to oncology patients. Scope of responsibility includes expert practitioner, educator/teacher, care management, consultant, nurse researcher, and nurse leader. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES: -Assesses, plans, implements and evaluates care for individual patients. -Initiates and documents appropriate interventions; evaluates and records patient/family response within practice protocol; prescribes drug therapies per protocol/physician consultation. -Assists physicians and performs procedures as needed; conducts patient assessments and provides physician with information prior to seeing the patient. -Assesses patient/family needs and provides education. -Promotes wellness and recommends community resources to meet patient and family needs. -Promotes/provides staff education. -Incorporates evidence-based research into clinical practice. -Knowledgeable of reimbursement procedures and documentation requirements. -Completes patient charge tickets. -Provides case management for oncology clients and their families. -Serves as a resource person for the nursing staff by providing information being studied in clinical trials. -May supervise the activities of clinical nursing and support staff. -May mentor new mid-level providers Qualifications MINIMUM QUALIFICATIONS: Nurse Practitioner - Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program. This position is entry-level and typically requires a minimum of 0-3 years post MSN experience, preferably in the chosen specialty. RN state licensure and current registration with the State Board of Nursing in practicing state. Current state licensure as a Nurse Practitioner. Current basic life support (BLS)/CPR certification required. OR Physician Assistant Successful completion of an accredited Physician Assistant program. Current NCCPA (National Commission on Certification of Physician Assistants) certification. Compensation will include an on-call stipend PLUS pay per visit for each visit completed.
