About Activate Care: At Activate Care, we’re on a mission to improve health equity and drive improved health outcomes across the country. Our Community Care Record platform, Care Link, enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs (HRSN). Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Our goal is simple: address individuals’ unmet HRSNs, increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend. Role Overview: This community-focused, Registered Nurse (RN) will travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. The role specializes in conducting timely, accurate assessments and documenting findings, then handing off to the health plan's care management team. This is assessment and documentation work, not ongoing care management. Responsibilities: Engage with members in their homes and other community settings to complete assessments, with mindfulness of the cultural and linguistic needs of each member. Perform required assessments on a timely basis as assigned under the contract, including Uniform Core Assessments (UCA), Functional Assessments (FA), and Comprehensive Assessments, evaluating each member's physical, cognitive, and functional status. Document assessments in the health plan's care management platform (GuidingCare) following the provided workflows and resource guides. Send a SOAP note to the assigned health plan Care Manager upon assessment completion. Submit accurate, complete documentation within 48 hours of each assessment and maintain a high completeness and quality standard as measured through the health plan's review and audit. Complete required training and ongoing training to nd maintain system access. Routinely travel to members' homes and, or community settings to conduct assessments. Report abuse, neglect, or exploitation of older adults and adults with disabilities as a mandated reporter, as required by state law. Follow field-safety practices for independent in-home visits and escalate clinical or safety concerns appropriately. Adhere to all applicable regulatory, privacy (HIPAA), NCQA, and care management standards Maintain licensure and or certifications Other duties as assigned. Qualifications & Skills: Active Registered Nurse (RN) or higher licensure (e.g., Nurse Practitioner) in good standing with the Commonwealth of Massachusetts. Associate of Science degree in nursing from an accredited program is required (BSN preferred). Valid driver's license, a personal vehicle, and verifiable insurance are required. Minimum of 1 year of clinical experience in a home health setting. Minimum of 1 year of experience completing assessments is preferred. Prior experience with dual-eligible populations, Medicaid, or Medicare managed care is preferred. Assessment-related certification (for example, RAC-CT) is preferred. Comfortable working independently in members' homes and managing a daily visit schedule. Ability to document accurately and efficiently in an electronic platform via a secure virtual desktop. Understanding of Medicare and Medicaid programs and the populations served. Strong interpersonal and communication skills to engage members and families. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of professional nursing care in Telemetry. The Registered Nurse is directly responsible and accountable for the care given to his/her patients in the Emergency Department. He / She communicates with the physician about the changes in the patient’s clinical condition including results of the diagnostic studies and symptomatology. Is capable of responding quickly and accurately to changes in the condition and/or response to treatment. Is knowledgeable about EMTALA regulations and reporting of child/adult abuse to appropriate agencies and Hospital Social Worker. Maintains patient privacy and confidentiality. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. Current and valid state RN License.2. Current BLS certificate upon hire and maintain current.3. Current ACLS certificate 30 days upon hire and maintain current. 4. Current PALS certificate and/or ENPC 30 days upon hire and maintain current. 5. Basic Arrythmia Interpretation within 30 days of hire. 6. Minimum of one year experience as a staff nurse RN in an acute care hospital setting; E.R. setting preferred.7. Certified Emergency Nurse (CEN) preferred.8. Bachelor of Science in Nursing (BSN) preferred. Facility Specific Requirements (facility may require items listed below): 1. In Paramedic Base Station ED: MICN within 12 months of hire and maintain current. 2. EDAP certified facilities: Completion of emergency pediatric course 1 and 2 within 6 months of hire.3. Non-OB facilities: NRP certificate 90 days upon hire and maintain current.4. Trauma Nurse Core Course (TNNC) within 12 months of hire required. Learn more at www.landmarkmedical.org. Responsibilities The PACU RN is accountable directly to the OR Charge Nurse and the Director of Surgical Services, and indirectly to the House Supervisor. The Registered Nurse is responsible for the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation for all patients within the PACU scope of services. The Registered Nurse also coordinates the post-operative care of the patient with the surgeon and anesthesiologist, directs and guides other personnel while maintaining standards of care in the PACU. He/She communicates with the physician about changes in the patient's clinical condition including hemodynamic monitoring, results of diagnostic studies and abnormal results and symptomatology. Works with other departments and co-workers to provide quality patient care across the continuum, efficient patient flow and unit maintenance and organization. Responsible for adherence to regulations, standards and hospital or unit policies and procedures. Responsible for participation in Performance Improvement activities, unit meetings and in-services. Acts as a resource for other personnel as appropriate. Qualifications Education and Work Experience 1. Current and valid state RN License.2. Current BLS (AHA) certificate upon hire and maintain current.3. ACLS (AHA) certificate 30 days upon hire and maintain current.4. PALS (AHA) certificate preferred.5. Previous Critical Care or PACU experience or minimum of one year on the job training.6. Critical Care RN (CCRN) preferred. 7. Bachelor of Science in Nursing (BSN) preferred.
