You will work the evening shift, 12p-12a and every other weekend, and rotating holidays. MDS Coordinator SNF Located in Far Rockaway, NY Salary: $110K to $130K range (based on experience) APPLY NOW!! Qualifications: Must have a current New York RN license Must have long-term care experience Must have current experience as an MDS Coordinator Must know MDS 3.0 Have a strong understanding of the MDS process Strong decision-making abilities and problem-solving skills Responsibilities: Conduct and coordinate the development and completion of the resident assessment (MDS) Maintain and periodically update written policies and procedures that implement MDS and care plan. Assist the resident in completing the care plan portion of the resident’s discharge plan. Develop and implement procedures with the Director of Nursing Services to inform all assessment team members of the arrival of newly admitted residents. Assist Facility directors and supervisors in scheduling the resident assessment and care plan meetings. Assist in determining appropriate treatment, selecting activities and exercises based on medical and social history of residents.
