Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. Provides personal grooming: i.e. hair, nails, shaving, personal clothing, mouth care, perineal care. Prepares laundry for pick-up and return. Checks resident's body for skin care: i.e. rash, any discoloration or edema. Feeding and Assisting with Feeding (when necessary) Provides calm, pleasant environment. Engages in cluster feeding while seated. Monitors daily intake and keeps nurses informed. Records food intake on Nutrition Work Sheet. Making Beds Uses recommended amount of linen and diapers. Disposes of linen in proper manner. Properly utilizes clean and dirty linen carts. Secures call bells to bed. Beds are made in proper manner. Knowledgeable in procedure for linen precaution. Safety of Residents: Restraints are to be properly applied and released and removed every two (2) hours for at least 15 minutes. Make sure, if restraint is used, that it was indicated on assignment and the proper type is applied. Restraints are checked at the beginning and end of each shift. Supervises ambulation of resident when needed. Proper attention is given that side rails are up on bed when needed. Completes dining room duty as posted by charge nurse. Is alert for spills on floor. Answers call bells as soon as possible. Makes rounds frequently. Reports need for repair and safety matters to charge nurse. To Ascertain Vital Signs Follows procedure in taking TPR correctly. Adheres to the proper procedure for cleaning thermometers/IVAC. Rehabilitation Assists in ambulation, transfers, ROM exercises and good body alignment where indicated. Uses adaptive devices properly. Cooperates with all rehab disciplines. Keeps accountability sheets up to date with signatures as required. Testing of Urine and Collecting of Specimens Has knowledge of proper procedure. Records results. Informs nurse of results. Assignments Receives daily report from nurse. Consults assignment for special duties and sets proper priorities for following through on assignment. Is familiar with and takes proper action in meeting physical needs of resident. Routinely checks and keeps in order all residents' belongings: i.e. clothing, glasses, hearing aid, prosthesis aid, adaptive devices, etc. Ensures that residents on turning schedule is positioned every two hours or as ordered, and documented. Ensures that residents on toileting schedule are assisted as needed, and documented. Resident is kept clean and dry. Gives assistance to co-workers when needed. Uses good handwashing techniques. Cooperation Works well with co-workers and supervisors. Maintains appropriate relationship with residents, families and peers. Gives assistance to other departments: i.e. recreation, when necessary. Communicates well with other staff members.
