Position Summary: We have an exciting opportunity to join our team as a Certified Lactation Registered Nurse (IBCLC Certification Required). In this role, the successful candidate The Certified Lactation Registered Nurse is a member of the health care team who is knowledgeable, skilled and experienced in lactation and breastfeeding management and whose primary focus is the provision of education, assistance and support to breastfeeding women and children Job Responsibilities: Performs a complete assessment of the breastfeeding mother and infant. Analyzes assessment information to identify concerns and/or problems experienced by the mother and infant. Implements a plan of care for the breastfeeding mother and infant. Considers the needs and behaviors of specific patient age groups and cultures. Manages the Breast Pump Discharge Program to assure all mothers have a pump at home upon once discharged. Promotes own professional growth and development. Serves as a support and resource for the education of other health professionals. Educates parents on all aspects of infant feeding and assures all patients have discharge resources to support infant feeding after discharge. Assures compliance with standards and regulations promulgated by all applicable agencies. In conjunction with nursing leadership assures the Donor Milk program practices are compliant as per regulation Participates as a member of the Women and C Services Quality and Performance Improvement Councils. Liaison for cross campus meetings with regard to breastfeeding and Baby friendly updates In conjunction with nursing leadership supports new initiatives related to breastfeeding Applies research findings that support lactating mothers and/or their breastfeeding babies. Minimum Qualifications: To qualify you must have a Must be a graduate of an Accredited Nursing Program. NY State RN License/BLS Certification Current Certification by the International Board of Lactation Consultant Examiners (I.B.L.C.E.). Lactation Consultant Course. Minimum of 2 years Certified Lactation RN experience. Evidence of excellent interpersonal skills, effective communication skills and creative problem solving ability.. Required Licenses: Registered Nurse License-NYS, Basic Life Support Cert Preferred Qualifications: Current Certification by the International Board of Lactation Consultant Examiners (I.B.L.C.E.). In conjunction with patient’s family and physician, develops and implements the Plan of Care based upon a comprehensive physical, psycho-social and environmental assessment. Provides skilled nursing care as described in the 485-Plan of Treatment, such as, but not limited to, wound care, injections, prepour/prefill of medications, disease management, medication management, etc.. Orients and supervises home health aide personnel in accordance with regulatory requirements and documents accordingly. Evaluates the effectiveness of interventions in accordance with the plan of care. Identifies the need for evaluation by other disciplines such as physical therapy, occupational therapy, MSW, speech therapy and nutrition. Observes signs and symptoms and changes in patient’s clinical, psychological and functional status. Consults with physician regarding changes in the treatment plan. Educates, counsels, supervises patients and caregiver relating to disease management and medical regime. Documents in HCHB Clinical Software all assessments, treatments and services provided and patient response to the treatment plan. Case manages respective caseload which includes, but is not limited to, ongoing communication with MD, case conferencing with supervisor, obtaining updates from interdisciplinary team, completion of interim orders, updating of patient profile, conducting recertification assessments, etc. Contacts physician to report, clarify and/or obtain orders for; medication changes/additions, precautions, treatment, changes in visit frequencies, additional services needed, requests for supplies and equipment, plans for discharge from a service or the agency. #5920
