Description The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Resource Utilization Ensures an appropriate level of care order is documented in the EMR for all admissions. Understands and adheres to the charging of supplies. Use of Technology Assures all electronic and paper documentation is complete. Utilizes the guardrails for the Alaris pump. Understands physiologic monitoring and the implications to drive changes to clinical management of the patient. Participates in CEOC activities for the advancement and optimization of the medical record as requested. Overview Department: Medical Surgical Telemetry Schedule: Full-Time, Days 12 Hour Shifts Hospital: Holy Family Medical Center- LTACH Location: Des Plaines, IL 60016 Responsibilities The Patient Care Assistant assists patients with activities of daily living, provides basic nursing care to all patients and assists in maintenance of a safe environment under the direction and supervision of the Registered Nurse in charge of the team and/or unit. Qualifications EDUCATION, EXPERIENCE, TRAINING 1. High School Diploma or equivalent. 2. Certified Nursing Assistant (CNA), Illinois approved Certification required. 3. Current Phlebotomy certification upon hire, preferred. 4. Current EKG certification upon hire, preferred. 5. Current BLS (AHA) within 30 days of hire and maintain current. 6. Possesses knowledge of Medical Terminology. 7. Minimum six months previous experience in an acute care hospital or physician’s office preferred. As the Afterhours Registered Nurse's your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends. Primary Responsibilities: Able to triage patient needs over the phone until a nurse can provide an in-person visit Responds appropriately and with urgency to crisis calls from patients and families afterhours Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassess them as needed, no less than every fifteen (15) days Documents problems, appropriate goals, interventions, and patient/family response to hospice care Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
