Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Description RN/Case Manager MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Responsibilities PRINCIPLES DUTIES AND RESPONSIBILITIES CLINICAL PRACTICE Assesses the health status of patients/families by means of comprehensive health history and physical examination. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. Documents findings and plan of care in the patient's medical record. Consults with physician according to established standards, policies, procedures, and protocols. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. Initiates referrals to specialty services and fosters continuity of care. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. Maintains patient/employee confidentiality in the management of information. Develops the plan of care based on the diagnoses and individual patient needs: Records, reports, and interprets the clients responses to the plan of care. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. PROFESSIONAL DEVELOPMENT Supports the mission, vision, philosophy and goals of the Department of Nursing and the MSHS. Demonstrates accountability for own nursing practice. Considers ethical issues of professional nursing practice and adheres to the code of ethics. Demonstrates a humanistic, kind, and caring attitude in the delivery of health care. Serves as a resource person to nurses and other health team members. Participates in peer review. Incorporates research findings in clinical practice. Participates in approved nursing and medical research studies. Keeps knowledge current by self-directed learning, literature review and attendance at educational programs. Participates in defining, maintaining, and interpreting standards of nursing practice. Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession. Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery. Seeks opportunities to enhance professional nursing practice and influences outcomes. Treats co-workers, patients and families with dignity and respect. KNOWLEDGE, SKILLS AND ABILITIES REQUIRED Ability to perform history taking. Ability to perform physical assessment techniques. Demonstrate and complete documentation of competency in specialty specific procedures according to established standards as part of initial orientation and annual performance review with collaborating physician or appropriately credentialed designee. Ability to interpret and apply principles of nursing research utilization. Ability to manage and coordinate a patient caseload. Work within a Collaborative Practice Agreement or Collaborative Practice Relationship Attestation as a member of the health care team in area of practice Physical stamina and manual dexterity to perform a variety of tasks and skills in area of assignment. Excellent interpersonal skills, effective communication skills, creative problem solving and excellent critical thinking and leadership Qualifications Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate. Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), The Neonatal Resuscitation Program (NRP) certificates as required by specialty. Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Communicates values, strategies, and objectives of department or division on a regular basis. Assigns accountabilities, delegates tasks and responsibilities, and plans, monitors and appraises job performance. 3. Develops, maintains and reconciles departmental budget with financial responsibility and oversight for department or division. 4. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes, or recommends changes to superiors as necessary. 5. In conjunction with Nursing, Medical and Quality Leadership, coordinates educational and performance improvement activities within a designated clinical specialty. 6. Develops and designs of action plans to support clinical performance and competency and provides support and educational resources to departments and services relative to nursing care delivery within the context quality assessment and improvement processes. 7. Establishes and implements short and long term goals, objectives, policies and operating procedures for high level of performance in nursing care delivery. 8. Coordinates and leads in the execution of day to day educational program activities, as appropriate to program objectives and areas of clinical expertise. 9. Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Master's Degree in Nursing is required. NP Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Certification in specialty area preferred.
