Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. Schedule · 5 days a week (Wed-Sun) 37.5 - 40 hours per week Respiratory Nurse (RN) We are searching for a highly motivated, flexible, organized, skilled respiratory therapist who has experience in a rehab center and possesses great communication skills to put the “therapy” into respiratory therapy. As a Post Acute and Long Term Care respiratory therapist, your role would involve providing specialized care and treatment to patients with cardio-pulmonary, respiratory conditions neurological disorders and physical conditions that manifest into respiratory disorders over an extended period. Your primary responsibility would be to assess, provide diagnostic testing, develop treatment plans, and provide treatment to our Skilled Nursing Facility population. Job Description ESSENTIAL JOB FUNCTIONS: 1. Provides patient treatments which meet patient needs and uses current treatment knowledge in accordance with facilities clinical procedures. This includes but is not limited to: · Performing accurate, comprehensive patient evaluations. · Developing an effective plan of treatment and obtaining appropriate approvals from a referring physician. · Communicating with supervisor and interdisciplinary team members regarding patient progress, problems, and treatment plans. · Instructing patient’s families and/or nursing staff in specified follow through programs. · Interviewing patients and families regarding previous level of functioning, lifestyle and current and future expectations for recovery. · Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice. · Evaluating patients within the required timeframe as per facility policies. · Provides patient care as appropriate and necessary, following the position description for the applicable clinical discipline in which licensure or certification is held. 2. Registered Nurse – Intensive Mobile Treatment Team (Full-Time) About the Program Intensive Mobile Treatment (IMT) is a trauma-informed, community-based behavioral health program that provides treatment, rehabilitation, and support services to individuals who have not been successfully served by traditional systems. IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings. Position Overview The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals. Key Responsibilities • Conduct psychiatric and physical health assessments for program participants. • Develop and coordinate care plans with the multidisciplinary treatment team. • Monitor participant health status and modify treatment plans as needed. • Administer medications, injections, vaccines, and perform phlebotomy as ordered. • Monitor vital signs and medication side effects and communicate findings to the treatment team. • Coordinate medical appointments and collaborate with psychiatrists and community physicians. • Provide treatment, rehabilitation, and recovery support services. • Advocate for participants and assist them in accessing community resources and services. • Maintain accurate clinical documentation, progress notes, and medical records. • Participate in team meetings, supervision, and required trainings. • Provide crisis intervention services on a rotating basis. Qualifications • Currently licensed as a Registered Professional Nurse (RN) by the New York State Education Department. • Minimum two (2) years of experience working as a Registered Nurse. • Minimum two (2) years of experience working with individuals with mental health conditions in a health or social services setting. • Strong written, verbal, and interpersonal communication skills. • Ability to assess situations, synthesize information, and make appropriate clinical decisions. • Eligible for full and unconditional participation in Medicaid and Medicare programs. • Computer proficiency including MS Word, Excel, PowerPoint, and electronic health systems. • Comfort providing services in community settings such as shelters, public spaces, and outreach environments. • Ability to travel within the community, including use of public transportation. • Bilingual Spanish preferred. • Must be fingerprinted and cleared by the New York State Justice Center.
