Obtain and review patient consents and ensure required documentation for examinations, procedures, testing, and treatment is complete and accurate. Collect, label, process, and prepare gynecologic, prenatal, and other specimens for laboratory testing in accordance with proper handling, documentation, and transport procedures. Coordinate patient care through referrals, imaging, laboratory services, prescriptions, appointment scheduling, orders, and follow-up activities. Communicate laboratory and diagnostic results to patients in accordance with provider direction and established protocols and ensure appropriate follow-up is completed. Monitor patients for adverse reactions to medications, immunizations, procedures, and treatments and promptly report concerns to the provider. Support preventive women's health initiatives, including reproductive health education, cancer screening, immunization review, and patient education. Provides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures and standards of care. The LVN/LPT Direct Care Staff shall work under the general direction of the Program Administrator. Shifts Available: Full-Time 1- Tues-Sat 3p-11p 1- Fri-Mon (Fri/Sat 7am to 3pm) & Sun/Mon ( 3pm to 11pm)- Split Shifts Expected starting wage range is $28.43 - $35.13 . Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. What You Bring to the Table (Must Have) High School Graduation or G.E.D. Graduation from an accredited Licensed Vocational Nurse (LVN) /Licensed Psychiatric Technician ( LPT) program and current licensure Current CPR/BLS certificate Six months of prior experience providing direct care and special services to persons with developmental disabilities Complete the required training for the Registered Behavior Technician (RBT) certification within 6 months of initial employment and obtain RBT certification within twelve months of initial employment or be a Qualified Behavior Modification Professional and maintain credential through employment Within one year of employment, all direct care staff shall complete both segments of Direct Support Professional (DSP) competency-based training course including the competency tests or pass the department-approved challenge tests applicable to both training segments. Proof of prior successful completion of both training segments and competency exams will meet this requirement Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
