A licensed professional nurse directly under the supervision of the Director of Nursing Services that provides care and services to residents in a long-term care setting. Proficiency in the English language including the ability to speak, read, write, and comprehend the language with minimal to no difficulty. • Valid CPR certification General Duties and Responsibilities: GENERAL • Provides nursing care as prescribed by physician/health care professional in accordance with the legal scope of practice, any Board of Licensing restrictions, and within established standards of care, policies, and procedures • Administers professional services and provide care consistent with allowing residents to attain or maintain his or her highest practicable physical, mental, and emotional well-being utilizing the nursing process of assessing, planning, implementing, and evaluating patient care CLINICAL • Admits, transfers, and discharges residents in accordance with policy and procedure • Orients new residents/families as necessary • Gives/receives nursing reports upon beginning and ending of assigned shift. Reviews daily patient reports and assignments with Nursing Assistants upon the beginning of the assigned shift. • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to assess and evaluate the resident’s physical, medical and emotional status and to implement or revise nursing interventions to the resident plan of care. • Records care information accurately, timely and concisely. Completes all required documentation including resident assessments, interventions, and patient response(s) in the medical record in accordance with policy. • Initiates, reviews, revises, and updates resident Plans of Care as indicated. Evaluates the patient’s progress toward specific goals and adjusts the nursing plan of care, as necessary. Receives and transcribe orders accurately from attending/alternate physician • Receive and transcribe orders accurately from attending/alternate physician • Assists in making rounds with the attending physician, as necessary. • Prepares / administers medications as ordered by the physician and within the legal scope of practice. • Completes medical treatments as indicated and ordered by the physician. • Evaluates resident response and reports adverse reactions to medications or treatments in accordance with the policy. • Assures that narcotic and controlled drugs are secure, inventory counts are accurate and narcotic register documentation is timely and accurate • Orders prescribed medications, supplies and equipment as established by the facility’s policies and procedures. • Requisitions and arranges for diagnostic and therapeutic services as ordered by the physician. • Informs the RN Supervisor or RN in charge of the facility and others as appropriate of resident status and related matters • Attends various facility meetings (care conferences, stand-up meetings, staff meetings, Medicare meetings, discharge plan meetings, etc.) as required. • Reports allegations of abuse or unusual occurrence per State and Federal Regulations and facility policy. Assists with a gathering of information relative to the event. • Initiates emergency procedures (including basic CPR) and provide emergency patient treatments • Participates in the facility’s infection control program for monitoring communicable and/or infectious disease among residents and staff. • Participate in procedures for reporting hazardous conditions or equipment. • Assists in orientation of newly assigned personnel as requested. • Assumes Charge responsibility following orientation period as requested. • Performs other position-related duties as assigned. • Supervise CNAs SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying, and making corrections as necessary. • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care. • Assist the Charge Nurse in revision and distribution of nursing assignments as needed. • Assist in the supervision and direction of nursing personnel. • Participate in disciplinary action and evaluation of nursing personnel. Position Overview The Lead Medical Assistant provides day-to-day leadership and clinical support for school-based health center operations. This role combines hands-on patient care with staff guidance, workflow coordination, immunization program support, and quality improvement activities. Working collaboratively with the Clinical Senior Manager, providers, and school partners, the Lead Medical Assistant helps ensure efficient clinic operations, consistent clinical practices, and a welcoming, patient-centered experience for students and families. Primary Responsibilities Team Leadership & Clinic Operations Provide day-to-day guidance and support to Medical Assistants, promoting teamwork, accountability, and consistent clinical practices. Assist the Senior Manager with staffing coordination, workflow management, and daily clinic operations. Support the onboarding and training of new staff on clinical workflows, procedures, and electronic systems, including NextGen, i2i, and CAIR. Coordinate Medical Assistant activities in the supervisor's absence, under the direction of the Clinical Senior Manager. Ensure clinic readiness by monitoring medical supplies, equipment, exam rooms, emergency carts, and adherence to safety standards. Reinforce compliance with Eisner Health policies, procedures, and established clinical protocols. Vaccine Program & Immunization Coordination Support the Vaccines for Children (VFC) program by monitoring vaccine inventory and usage, coordinating vaccine orders, and maintaining accurate documentation. Collaborate with the CAIR Coordinator to ensure immunization records are accurate, historical vaccine information is entered correctly, and registry updates are completed. Coordinate immunization reminder and recall activities, patient follow-up, and the availability of current vaccine education materials. Support vaccine inventory management, immunization reporting, and compliance with applicable program requirements. Clinical Care & Patient Services Perform Medical Assistant duties, including obtaining vital signs and patient histories, preparing patients for examinations, assisting providers, administering medications and immunizations within the scope of practice, and performing blood draws and specimen collection. Maintain accurate electronic health records, verify patient documentation and consents, and ensure orders, test results, and billing-related services are completed. Provide patient education, reinforce provider instructions, monitor patient responses to treatment, and promptly report clinical concerns. Maintain clean, organized, and adequately stocked clinical spaces; monitor equipment and medication logs; and support referrals, scheduling, and patient follow-up. Data, Reporting & Quality Improvement Use electronic health systems and Microsoft Excel and PowerPoint to track clinic activities, analyze data, prepare reports, and present findings. Support immunization reporting, inventory tracking, and quality improvement initiatives. Patient Experience & School Community Engagement Build positive relationships with students, families, school faculty, and community partners. Promote a welcoming, inclusive, and culturally responsive care environment. Support front office operations, including registration, demographic updates, insurance screening, payments, appointment scheduling, and patient inquiries. Provide front and back office coverage as needed to support clinic operations and continuity of care. Required Qualifications Current Medical Assistant certification from a Medical Board of California-approved certifying organization required. At least two years of Medical Assistant experience in an outpatient setting, including both front office and back office responsibilities with adult and adolescent patients. Current CPR certification. Family Planning Health Care Worker certification, or willingness to complete the certification by the next available class or within the required timeframe. Bilingual proficiency in English and Spanish. Demonstrated knowledge of the Vaccines for Children (VFC) program. Experience with private vaccine programs alone does not meet this requirement. Experience with blood draws and specimen collection. Strong computer skills and proficiency with Microsoft Excel and PowerPoint. Ability to organize information, interpret data, prepare reports, and present findings. Ability to work flexible hours and travel between school-based clinic locations as needed. Ability to maintain patient confidentiality and work effectively with students, families, healthcare professionals, and school personnel. Preferred Qualifications Previous leadership, lead Medical Assistant, or team coordination experience. At least one year of experience in a family practice setting. Familiarity with CAIR, i2i, vaccine inventory management, and immunization reporting. Experience working in pediatrics, adolescent health, school-based health, or community health settings. Join our team at Lynwood Healthcare Center, a 99-bed facility POSITION SUMMARY The purpose of your job position is to provide each resident with routine daily nursing care in accordance with the residents assessment plan, with current federal, state and local standards and regulations that govern the facility, and as directed by your supervisors. The Certified Nurse Assistant will treat each resident with kindness, dignity and respect. They will knock before entering each resident’s room. They will refer to each resident by their name. They will be courteous to families and visitors. They will maintain a positive and calm disposition. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, staff and any governmental agency personnel. A Certified Nursing Assistant CNA is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. Certified Nursing Assistant CNA GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.
