Care giver performance, standard of care and current care plan will be evaluated, documented and communicated to Client Care Manger. · Conducts client caregiver introductions. · Document care / issues/ events in ClearCare and in client / caregiver files. · Conducts client and caregiver in home training visits on a predetermined schedule for the purposes of care plan compliance and the evaluating quality of care and going forward care plan updates. · Acts as a liaison between RAH caregiver, RAH clients, client families, office staff, and outside contacts. · Pro-actively learns the scheduling role and is able to actively back up this position in times of vacation or illness to the primary scheduler. · Actively engages in training and new hire orientation when required. · Protect company records and property. · Participate in educational opportunities in health care.
